摘要
目的 探讨基于3D技术分析颅内动脉瘤与周围血管的解剖关系的价值。方法 纳入我院就诊的45例颅内动脉瘤患者的影像资料,所有患者均进行CTA三维影像重建,随后将原始图像数据导入3D精准分析系统,建立3D颅内动脉瘤模型,并进行3D打印,获取颅内动脉瘤与周围血管走向、角度之间解剖关系的三维立体图像,使用红、蓝、绿等染色标记,以达到最佳的显示效果,并使用分割工具进行任意角度的分割和旋转,以测量颅内动脉瘤的高度、宽度以及观察瘤体与周围颅骨及血管解剖关系。结果 在45例动脉瘤中,瘤体长度为(5.33±1.15)mm、瘤颈宽度(3.21±1.05)mm、动脉瘤倾斜角(105.23±5.98)°、垂直高度与瘤颈宽比(AR)为1.26±0.47、动脉瘤高度与血管径比(SR)为3.21±1.14、面积比3.22±1.06,其中,35例处于分叉处的颅内动脉瘤在动脉瘤大小、瘤颈宽度、动脉瘤高度、动脉瘤高度与血SR、AR、血管角度、血管管径、冠状面朝向和横断面朝向等方面与其他部位动脉瘤(n=10)的差异均有统计学意义(P<0.05)。结论 3D打印技术能更直观清晰地观察颅内动脉瘤与周围血管、颅骨等结构的解剖关系,有利于介入手术的顺利开展。
Objective To evaluate the anatomical relationship between intracranial aneurysms and peripheral blood vessels based on 3D technique.Methods In our hospital data of 45 cases of patients with intracranial aneurysm image,all patients were performed CTA three-dimensional image reconstruction,then the original image data import 3 d precision analysis system,set up 3 d model of intracranial aneurysms,and 3 d printing,for intracranial aneurysm and its around vessel towards,the Angle of the anatomical relationship between 3 d images,Red,blue,green and other staining markers were used to achieve the best display effect,and segmentation tools were used to segment and rotate at any Angle to measure the height and width of intracranial aneurysms and observe the anatomical relationship between the tumor and the surrounding skull and blood vessels.Results In 45 aneurysms,The average length of the tumor was(5.33±1.15)mm,the width of the tumor neck was(3.21±1.05)mm,the inclination angle of the aneurysm was(105.23±5.98)°,the ratio of the vertical height to the width of the tumor neck(AR)was 1.26±0.47,the ratio of the aneurysm height to the diameter of the blood vessel(SR)was 3.21±1.14,and the area ratio was 3.22±1.06,there were statistically significant differences between 35 cases of intracranial aneurysms at the fork and other aneurysms(n=10)in aneurysm size,aneurysmal neck width,aneurysm height,aneurysm height and blood SR,AR,vascular angle,vascular diameter,coronal plane orientation and cross-sectional orientation(P<0.05).Conclusion 3D printing technology can more intuitively and clearly observe the anatomical relationship between intracranial aneurysm and the surrounding blood vessels,skull and other structures,which is conducive to the smooth development of interventional surgery.
