摘要
目的:对比反复种植失败(RIF)患者采用生长激素(GH)与粒细胞集落刺激因子(G-CSF)两种药物分别宫腔灌注后对子宫内膜容受性及冻融胚胎移植结局的影响。方法:采用对照研究的设计,选择2019年9月至2021年1月在昆明医科大学第二附属医院生殖医学科行冷冻胚胎移植的RIF患者118例,随机分为3组,GH组(n=40)接受人重组GH宫腔灌注治疗,G-CSF组(n=39)接受重组人G-CSF宫腔灌注治疗,空白对照组(n=39)不接受治疗。比较3组子宫内膜容受性及冻融胚胎移植结局。结果:3组患者妊娠结局对比分析后发现,仅胚胎种植率在GH组与对照组之间差异有统计学意义(P<0.05)。另外,GH组的子宫内膜血流参数的搏动指数(PI)、收缩期峰值流速/舒张期流速(S/D)两个值在内膜转化日低于对照组,差异有统计学意义(P<0.05);G-CSF组的蠕动波幅度较另外两组减小,差异有统计学意义(P<0.05)。结论:用GH宫腔灌注提高了RIF的胚胎种植率;GH宫腔灌注可能通过降低子宫内膜血流的PI、S/D改善RIF患者内膜的血流灌注;G-CSF宫腔灌注使RIF患者的子宫内膜蠕动波幅度显著减小。
Objective:This study aim to compare the effects of intrauterine infusion of growth hormone(GH)and granulocyte colony-stimulating factor(G-CSF)on endometrial receptivity and frozen embryo transfer outcome in patients with recurrent implantation failure(RIF).Methods:118 patients with RIF who underwent frozen embryo transfer in the Department of reproductive medicine of the Second Affiliated Hospital of Kunming Medical University from September 2019 to January 2021 were selected.and randomly divided into three groups.GH group(n=40)received intrauterine perfusion of human recombinant growth hormone,G-CSF group(n=39)received intrauterine stimulation of recombinant human granulocyte colony,and blank control group(n=39)did not receive treatment.Endometrial receptivity and frozen embryo transfer outcomes of the three groups were compared and analyzed.Results:The embryo implantation rate was significantly different between GH group and control group(P<0.05).The pulsatility index(PI)and systolic peak velocity/diastolic velocity(S/D)of endometrial blood flow parameters in GH group were lower than those in the other two groups on the day of endometrial transformation(P<0.05);The peristaltic amplitude of G-CSF group was significantly lower than that of the other two groups(P<0.05).Conclusions:The implantation rate of RIF was increased by intrauterine infusion of GH.GH intrauterine perfusion may improve the endometrial blood flow perfusion in patients with RIF by reducing PI and S/D of endometrial blood flow.Intrauterine infusion of G-CSF could significantly reduce the amplitude of endometrial peristalsis wave in patients with RIF,which may contribute to embryo implantation.
