摘要
目的观察麻芍平喘汤治疗儿童支气管哮喘(以下简称哮喘)急性发作期冷哮证的疗效及对肺功能的影响。方法将70例哮喘急性发作期冷哮证患儿按照随机数字表法分为2组。对照组35例予吸入用布地奈德混悬液、硫酸特布他林雾化吸入用溶液及吸入用复方异丙托溴铵溶液治疗,观察组35例在对照组治疗基础上予麻芍平喘汤治疗,2组均治疗2周。比较2组咳嗽、气促、哮鸣音消失时间及临床疗效,比较2组治疗前后中医证候评分及肺功能指标第1 s用力呼气容积(FEV_(1))、FEV_(1)/用力肺活量(FEV_(1)/FVC)、最大呼气流量(PEF)变化情况。结果观察组咳嗽、气促、哮鸣音消失时间均短于对照组(P<0.05)。治疗后,2组中医证候各项评分均较本组治疗前降低(P<0.05),且观察组均低于对照组(P<0.05)。治疗后,2组FEV_(1)、FEV_(1)/FVC、PEF均显著高于本组治疗前(P<0.05),且观察组均显著高于对照组(P<0.05)。观察组总有效率97.14%(34/35),对照组总有效率82.86%(29/35),观察组总有效率高于对照组(P<0.05)。结论麻芍平喘汤治疗儿童哮喘急性发作期冷哮证能缩短咳嗽、气促等症状消失时间,降低中医证候积分,改善肺功能,提升近期预后。
Objective To observe the curative effect of MashaoPingchuan Decoction for children with acute asthma attack(cold asthma)and how it impacts on pulmonary function.Methods Totally 70 children with acute asthma attack(cold asthma)were selected randomly divided into the control group and the observation group,35 cases in each group.All patients were managed by budesonide suspension liquid,terbutaline sulfate nebuliser solution and compound ipratropium bromide solution for inhalation,and those in observation group were additionally treated with MashaoPingchuan Decoction.Treated for 2 weeks,aiming to compare the disappearance time of wheezing sound,cough and panting,clinical efficacy,traditional Chinese medicine(TCM)symptom scores,pulmonary function index(Forced expiratory volume in one second[FEV_(1)],FEV_(1)/forced vital capacity[FVC],peak expiratory flow[PEF]).Results After treatment,the disappearance time of wheezing sound,cough and panting was decreased(P<0.05)in the observation group relevant to the control group;TCM symptom scores in groups were decreased(P<0.05),which were decreased in the observation group compared with the control group(P<0.05);FEV_(1),FEV_(1)/FVC,PEF in groups were notably increased(P<0.05),which in the observation group were significantly increased(P<0.05).The overall effective rate was 97.14%(34/35)in the observation group,higher than that of 82.86%(29/35)in the control group(P<0.05).Conclusion For children with acute asthma attack(cold asthma),MashaoPingchuan Decoction can shorten the disappearance time of cough and panting,reduce TCM syndrome scores,improve pulmonary function and short-term prognosis.
