摘要
经皮穿刺椎间孔镜术(percutaneous transforaminal endoscopy discectomy,PTED)可治疗腰椎间盘突出症,具有减少创伤、快速恢复和较少并发症等特点,此手术早期使用全身麻醉来保证镇痛效果,但术中若刺激神经根亦或损伤神经根,由于患者处于麻醉状态且无法及时反馈,可能会造成难以弥补的后果。为了安全,目前此类手术常选择局部麻醉。右美托咪定(dexmedetomidine,Dex)是高选择性的新型α2肾上腺素能受体激动剂,具有镇静、镇痛和抗焦虑效应,常与各种临床镇痛药复合应用于麻醉,在镇静、镇痛中发挥协同作用。近年也常将其应用于经皮穿刺椎间孔镜手术的麻醉,来改善镇静、镇痛的同时提高治疗的有效性和安全性。
Percutaneous transforaminal endoscopy discectomy(PTED)is effective in the treatment of lumbar disc herniation,which is characterized by less trauma,quick recovery and fewer complications.With the gradual updating of surgical methods,Intervertebral foramen in the mirror surgery anesthesia scheme also gradually varied,early anesthesia scheme for general anesthesia,general anesthesia can guarantee the analgesic effect of perfect,but if the surgeon during the operation stimulate the nerve root heavier or nerve root injury,because the patient is under anesthesia,do not give feedback in time,may lead to irreversible consequences.In order to ensure the safety of operation,local anesthesia is the most widely used anesthetic plan in clinic.Dexmedetomidine(dexmedetomidine,Dex),as a novel and highly selectiveα2 adrenergic receptor agonist,has sedative,analgesic and anti-anxiety effects,and is often used in anesthesia in combination with various clinical analgesics,which can play a synergistic effect of sedation and analgesia.In recent years,it is often used in percutaneous transforaminal surgery anesthesia to improve sedation,analgesia and improve the effectiveness and safety of treatment.
