摘要
目的:探讨MindrayBC-6800全自动血细胞计数分析仪白细胞散点图异常与血小板聚集报警信息相关性,了解白细胞散点图异常对血小板真性聚集的提示作用。方法:回顾性分析2017年6月—2020年12月贵阳市第四人民医院609例住院患者送检的血常规样本,按血小板直方图的不同报警信息,将MindrayBC-6800全自动血细胞计数分析仪报警信息为“血小板聚集”的420例血常规标本作为血小板聚集组,将报警信息为“血小板聚集/血小板直方图异常”的189例血常规标本作为血小板聚集/血小板直方图异常组。分析其白细胞散点图特点,并与人工镜检做比较。结果:以人工镜检为金标准判断,MindrayBC-6800全自动血细胞计数分析仪“血小板聚集”阳性报警的420例标本中,镜检血小板聚集399例,真阳性率为95.00%;“血小板聚集/血小板直方图异常”阳性报警的189例标本中,镜检32例聚集,真阳性率为16.93%。血小板聚集报警标本共609例,镜检血小板聚集431例,阳性率70.77%。此部分标本白细胞散点图在非白细胞散点区出现异常。大血小板增多157例,比率25.78%,此部分标本白细胞散点图正常。结论:MindrayBC-6800全血细胞计数分析仪分析血小板聚集时,存在一定的不足,需要结合人工显微镜检查,方能得出准确计数结果。血小板聚集报警提示信息可作为血小板聚集和异常的筛选指标之一,联合白细胞散点图能快速判断和筛选真性血小板聚集,准确计数血小板结果必须进行人工推片显微镜检查。
Objective To explore and analyze the correlation between abnormal white blood cell scatter diagram and platelet aggregation alarm information of Mindray bc-6800 automatic blood cell count analyzer used in this study,and to understand the suggestive effect of abnormal white blood cell scatter diagram on true platelet aggregation.Methods The blood routine samples of 609 inpatients in Guiyang Fourth People’s Hospital from June 2017 to December 2020 were analyzed retrospectively.According to the different alarm information of platelet histogram,420 blood routine samples with the alarm information of Mindray bc-6800 automatic blood cell count analyzer as“platelet aggregation”were divided into platelet aggregation group and platelet aggregation group,189 blood samples with alarm information of“platelet aggregation/abnormal platelet histogram”were divided into platelet aggregation/abnormal platelet histogram group,.The characteristics of leukocyte scattergram were analyzed and compared with artificial microscopy.Results Judging by the gold standard of artificial microscopic examination,420 samples of Mindray bc-6800 automatic blood cell count analyzer gave a positive alarm of“platelet aggregation”,399 samples of platelet aggregation were examined by microscope,and the true positive rate was 95.00%.The positive alarm of“platelet aggregation/abnormal platelet histogram”was 189 samples,and 32 samples were clustered by microscopic examination.The true positive rate was 16.93%.There were 609 alarm samples of platelet aggregation,including 431 cases of platelet aggregation by microscopic examination,with a positive rate of 70.77%.There were 157 cases of large thrombocytosis,with a rate of 25.78%.The white blood cell scatter diagram of some samples was normal.Conclusion Mindray bc-6800 whole blood cell count analyzer has some shortcomings in analyzing platelet aggregation.It needs to be combined with artificial microscope to obtain accurate counting results.Platelet aggregation alarm information can be used as one of the screening indicators of platelet aggregation and abnormalities.Combined with leukocyte scatter diagram,it can quickly judge and screen the true platelet aggregation.The accurate platelet count results must be examined by manual push film microscope.
