Correlation between intestinal microorganisms and their metabolites and heart failure

摘要 目的研究肠道微生物及其代谢产物与心力衰竭的相关性。方法选择我院2020年12月至2021年12月纳入的心力衰竭患者45例作为研究组,另选择同一时间纳入的37名健康体检者作为对照组,分别观察两组肠道微生物及其代谢产物情况,分析其与心力衰竭的相关性。结果心功能Ⅲ级的拟杆菌、双歧杆菌、乳杆菌低于心功能Ⅱ级、对照组,但酵母样真菌、大肠埃希菌高出心功能Ⅱ级、对照组(P<0.05)。心功能Ⅲ级的B型钠尿肽(BNP)、C反应蛋白(CRP)、氧化三甲胺(TMAO)高出心功能Ⅱ级、对照组,但左室射血分数(LVEF)低于心功能Ⅱ级、对照组(P<0.05)。拟杆菌、双歧杆菌、乳杆菌与BNP、CRP呈现负相关性,但与LVEF呈现正相关性;酵母样真菌、大肠埃希菌、TMAO与BNP、CRP呈现正相关性,但与LVEF呈现负相关性(P<0.05)。结论肠道微生物及其代谢产物与心力衰竭存在密切关系,肠道菌群失衡在疾病发生发展中意义重大,应受到临床关注。 Objective To study the correlation between intestinal microorganisms and their metabolites with heart failure.Methods 45 patients with heart failure who were included in our hospital from December 2020 to December 2021 were selected as the study group,and 37 healthy people who were included in the same time were selected as the control group.The intestinal microorganisms and their metabolites were observed in the two groups,and their correlation with heart failure was analyzed.Results Bacteroides,bifidobacteria and Lactobacillus in cardiac function classⅢwere lower than those in cardiac function classⅡand control group,but yeast like fungi and Escherichia coli were higher than those in cardiac function classⅡand control group(P<0.05).The levels of B-type natriuretic peptide(BNP),C-reactive protein(CRP)and trimethylamine oxide(TMAO)in gradeⅢcardiac function group were higher than those in gradeⅡcardiac function group and control group,but the left ventricular ejection fraction(LVEF)was lower than that in gradeⅡcardiac function group and control group(P<0.05).Bacteroides,Bifidobacterium and lactobacillus were negatively correlated with BNP and CRP,but positively correlated with LVEF.Yeast like fungi,Escherichia coli and TMAO were positively correlated with BNP and CRP,but negatively correlated with LVEF(P<0.05).Conclusion there is a close relationship between intestinal microorganisms and their metabolites and heart failure.The imbalance of intestinal flora is of great significance in the occurrence and development of the disease and should be paid attention to clinically.
