摘要
目的建立重庆地区健康成人血清铁蛋白参考区间。方法选取重庆地区1996例健康成人检测血清铁蛋白,比较男女性的血清铁蛋白浓度,使用非参数排序法计算P_(2.5)~P_(97.5)区间,建立重庆地区健康成人血清铁蛋白参考区间,并进行验证。结果健康成年男性和健康成年女性血清铁蛋白测定结果均为偏态分布,应当采用百分位数法进行进一步分析。经秩和检验分析,健康成年男性血清铁蛋白浓度明显高于健康成年女性,差异具有统计学意义[334.50(225.40,488.70)ng/ml VS 79.17(39.88,133.10)ng/ml,P<0.01]。将同性别血清铁蛋白浓度按年龄分段并同上一年龄段进行比较,≥61岁健康成年男性的血清铁蛋白浓度低于51~60岁男性,差异具有统计学意义(P<0.01)。51~60岁健康成年女性的血清铁蛋白浓度高于41~50岁女性,差异具有统计学意义(P<0.01)。女性各年龄段的血清铁蛋白浓度均明显低于男性,差异具有统计学意义(P<0.01)。可知,重庆地区健康成年不同性别的血清铁蛋白浓度有差异,相同性别不同年龄段的血清铁蛋白浓度也有差异,所以重庆地区健康成年参考区间应针对不同性别和年龄段来进行设置。将无统计学意义的年龄段进行合并,得出血清铁蛋白不同性别及年龄段参考区间:男性:18~60岁为88.93~1139.35 ng/ml,≥61岁为34.10~854.42 ng/ml;女性:18~50岁为6.17~260.73 ng/ml,≥51岁为30.59~500.45 ng/ml。统计文献可知,血清铁蛋白在不同地区,不论检测方法是否相同,其参考区间并不一致。选取2018年1月~2019年3月在健康体检中心体检的体检人员进行参考区间验证,按照CLSIC28-A3文件剔除离群值,经验证,本次血清铁蛋白参考区间通过验证。结论重庆地区健康成人男、女血清铁蛋白参考区间与文献报道存在差异,重庆地区应建立适合本地区人群的血清铁蛋白参考区间。
Objective To investigate the reference interval for serum ferritin in healthy adults in Chongqing.Methods The serum ferritin of 1996 healthy adults were determined in Chongqing.Serum ferritin levels were compared between males and females,and the P_(2.5)~P_(97.5) interval was calculated using the nonparametric ranking method to establish a reference interval for serum ferritin in healthy adults in Chongqing and validated it.Results The results of serum ferritin measurements in healthy adult males and healthy adult females were skewed,and the percentile method should be used for further analysis.After rank sum test analysis,the serum ferritin concentration of healthy adult males was significantly higher than that of healthy adult females,and the difference was statistically significant{334.50(225.40,488.70)ng/ml VS 79.17(39.88,133.10)ng/ml,P<0.01}.The serum ferritin concentration of the same gender was divided into age groups and compared with the previous age group.The serum ferritin concentration of healthy adult males aged≥61 years was lower than that of males aged 51-60 years,and the difference was statistically significant(P<0.01).The serum ferritin concentrations in healthy adult female aged 51-60 years were higher than those in females aged 41-50 years,and the differences were statistically significant(P<0.01).The serum ferritin concentrations in female were significantly lower than those in males at all ages,and the difference was statistically significant(P<0.01).The serum ferritin concentration of healthy adults in Chongqing area varies among different genders and among different age groups of the same gender,so the reference interval of healthy adults in Chongqing should be set for different genders and age groups.The non-statistically significant age groups were combined to derive reference intervals for serum ferritin by gender and age:males:88.93-1139.35 ng/ml for 18-60 years of age and 34.10-854.42 ng/ml for≥61 years of age;female:6.17-260.73 ng/ml for 18-50 years of age and 30.59-500.45 ng/ml for≥51 years of age.Statistical literature showed that serum ferritin had inconsistent reference intervals in different regions,regardless of whether the assay was the same.Medical examiners who were examined at health screening centers from January 2018 to March 2019 were selected for reference interval validation,and outliers were excluded according to the CLSIC28-A3 document,and it was verified that this serum ferritin reference interval passed validation.Conclusion The serum ferritin reference intervals for healthy adults in Chongqing for both males and females differ from those reported in the literature,and the laboratories in Chongqing should establish its own serum ferritin reference interval.
