摘要
目的 探讨多囊卵巢综合征(PCOS)患者应用来曲唑和氯米芬的治疗效果。方法 选取2016年1月至2020年12月东莞东华医院收治的PCOS患者86例作为研究对象,随机分为观察组与对照组,各43例。对照组应用氯米芬,观察组应用来曲唑。比较两组用药前卵泡期(月经第3~5天)以及用药后人绒毛膜促性腺激素(HCG)扳机日的性激素水平,治疗后不良反应发生率,卵泡发育情况,子宫内膜厚度。结果 观察组治疗后睾酮(T)水平低于对照组,黄体生成素(LH)、雌二醇(E2)、促卵泡成熟激素(FSH)和子宫内膜厚度均高于对照组,差异有统计学意义(P<0.05);观察组优势卵泡数目低于对照组,窦卵泡数量高于对照组,差异有统计学意义(P<0.05);观察组瘙痒、腹泻、头痛、便秘、恶心发生率低于对照组(P<0.05);观察组妊娠率、月经周期恢复率、排卵率高于对照组,卵巢肿瘤发生率低于对照组,差异有统计学意义(P<0.05)。结论 在PCOS治疗中,相较于氯米芬,来曲唑效果更佳,可以有效改善患者性激素水平、子宫内膜厚度和卵泡情况,降低不良反应及卵巢囊肿发生率,提高妊娠率、排卵率、月经周期恢复率。
Objective To investigate the effect of clomiphene and letrozole in the treatment of polycystic ovary syndrome(PCOS).Methods 86 patients with PCOS admitted to Dongguan Donghua Hospital from January 2016 to December 2020 were randomly divided into observation group and control group,with 43 cases in each group.The control group was treated with clomiphene and the observation group was treated with letrozole.The levels of sex hormones in the follicular period before medication(3~5 days of menstruation) and HCG days after medication,the incidence of adverse reactions after treatment,follicular development,and endometrial thickness in the two groups were compared.Results After treatment,the level of testosterone(T) in the observation group was lower than that in the control group,and the levels of luteinizing hormone(LH),estradiol(E2),follicle stimulating hormone(FSH)and endometrial thickness in the observation group were higher than those in the control group(P0.05);the pregnancy rate,menstrual cycle recovery,and ovulation rate in the observation group were higher than those in the control group,the incidence of tumors was lower than that of the control group(P<0.05).Conclusion In the treatment of polycystic ovary syndrome,letrozole is more effective than clomiphene.It can effectively improve the sex hormone level,endometrial thickness and follicle condition of patients,and reduce the incidence of adverse reactions and ovarian cysts.It is worthy of promotion and application to improve the pregnancy rate,ovulation rate,and menstrual cycle recovery.
