摘要
目的分析连续性血液净化治疗脓毒症休克并发急性肾损伤的临床效果。方法将2018年6月—2021年5月广东省惠州市第六人民医院肾内科收治的50例脓毒症休克并发急性肾损伤患者为研究对象,按随机双盲法分为两组,各25例。对照组进行常规治疗,治疗组在对照组基础上进行连续性血液净化治疗,对比两组ICU住院时间、病死率、治疗后第2天肾功能指标及治疗前后急性生理学及慢性健康状况评分系统(APACHEⅡ)评分、序贯器官衰竭估计(SOFA)评分、生命体征指标。结果治疗组ICU住院时间(12.10±2.06)d短于对照组,病死率0.00%低于对照组,差异有统计学意义(t=5.169,χ^(2)=4.735,P<0.05)。治疗后第2天,治疗组BUN、Scr水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,治疗组APACHEⅡ评分、SOFA评分、HR均低于对照组,而MAP、CVP均高于对照组,差异有统计学意义(P<0.05)。结论连续性血液净化治疗脓毒症休克并发急性肾损伤可有效改善患者肾功能、生命体征,对其预后改善有重要意义。
Objective To analyze the clinical effect of continuous blood purification in the treatment of septic shock complicated with acute kidney injury.Methods A total of 50 patients with septic shock complicated by acute kidney injury who were admitted to the department of Nephrology of the Sixth People's Hospital of Huizhou City in Guang⁃dong Province from June 2018 to May 2021 were selected as the research objects.They were randomly divided into two groups,with 25 cases in each group.The control group was given routine treatment,and the treatment group was given continuous blood purification treatment on the basis of the control group.The ICU hospitalization time,mortal⁃ity rate,renal function indexes on the 2nd day after treatment,and Acute Physiology And Chronic Health Status Scor⁃ing System(APACHEⅡ)score,Sequential Organ Failure Estimation(SOFA)score,vital signs indicators before and after treatment were compared between the two groups.Results The ICU stay in the treatment group was(12.10±2.06)d shorter than that in the control group,and the mortality rate was 0.00%lower than that in the control group,and the difference was statistically significant(t=5.169,χ^(2)=4.735,P<0.05).On the 2nd day after treatment,the lev⁃els of BUN and Scr in the treatment group were lower than those in the control group,and the difference was statisti⁃cally significant(P<0.05).After treatment,APACHEⅡscore,SOFA score and HR in the treatment group were lower than those in the control group,while MAP and CVP were higher than those in the control group,and the differ⁃ences were statistically significant(P<0.05).Conclusion Continuous blood purification in the treatment of septic shock complicated with acute kidney injury can effectively improve the renal function and vital signs of patients,and it is of great significance to improve the prognosis.
