全飞秒激光小切口角膜基质内透镜取出术对屈光不正患者的影响 被引量:5

Influence of Full Femtosecond Laser Small Incision Lenticule Extraction on Patients with Ametropia

摘要 目的探讨全飞秒激光小切口角膜基质内透镜取出术(SMILE)对屈光不正患者的疗效.方法回顾性分析146例(292眼)屈光不正患者的临床资料,按手术方式分为对照组(n=70,140眼)和观察组(n=76,152眼),对照组采用飞秒激光制瓣-激光原位角膜磨镶术(FS-LASIK)治疗,观察组采用SMILE治疗,比较两组眼表疾病指数(OSDI)、最佳矫正视力(BCVA)、裸眼视力(UCVA)、角膜荧光染色素染色评分、泪河高度(TMH)、非侵入式泪膜破裂时间(NIKBUT)、脂质层厚度、睑板腺功能评分及眼红指数(HI).结果术后1周、术后1个月和3个月,观察组OSDI评分低于对照组,NIKBUT高于对照组(P<0.05).术后1周、术后1个月和3个月,两组角膜荧光染色素染色评分、TMH、脂质层厚度、睑板腺功能评分、HI、BCVA(logMAR)、UCVA(logMAR)比较差异无统计学意义(P>0.05).结论FS-LASIK和SMILE均可改善屈光不正患者视力,但SMILE对角膜知觉损伤更小,且对眼表影响较小. ObjectiveTo investigate the curative effect of full femtosecond laser small incision lenticule extraction(SMILE)on ametropia.MethodThe clinical data of 146 patients(292 eyes)with ametropia was retrospectively analyzed.The patients were divided into control group(n=70,140 eyes)and observation group(n=76,152 eyes)according to the surgical method.Patients in the control group were treated with femtosecond laser-assisted laser in situ keratomileusis(FS-LASIK),and those in observation group were treated with SMILE.The ocular surface disease index(OSDI),best corrected visual acuity(BCVA),uncorrected visual acuity(UCVA),corneal fluorescein staining score,tear meniscus height(TMH),non-invasive keratograph tear film breakup time(NIKBUT),lipid layer thickness,meibomian gland function score and hyperaemia index(HI)were compared between the two groups.ResultsAt 1 week,1 month and 3 months after operation,the OSDI scores of observation group were lower than those of control group,and NIKBUT was longer than that of control group,the difference was statistically significant(P<0.05).There were no significant differences in corneal fluoresce in staining,TMH,lipid layer thickness,meibomian gland function score,HI,BCVA(logMAR)and UCVA(logMAR)between the two groups at 1 week,1 month and 3 months after operation(P>0.05).ConclusionBoth FS-LASIK and SMILE can improve visual acuity of patients with ametropia,but SMILE causes less damage to corneal perception and less impact on ocular surface.
