2019-2020年深圳市大鹏新区柯萨奇病毒A6型流行及其VP1基因特征分析 被引量:5

Analysis of coxsackievirus type A6 prevalence and its VP1 gene characteristics in Dapeng New district, Shenzhen, 2019-2020

摘要 目的 了解2019-2020年深圳市大鹏新区手足口病(hand-foot-mouth disease, HFMD)病例中柯萨奇病毒A6型(CVA6)流行情况及VP1区基因特征,为HFMD的预防和控制提供科学依据。方法 收集临床诊断为HFMD病人的肛拭样本,采用实时荧光RT-PCR方法检测病毒核酸,以RT-PCR法扩增CVA6 VP1全长基因并测序,利用生物信息软件在分子水平分析毒株的进化特征。结果 在检出的156份肠道病毒阳性样本中,CVA16、CVA6、CVA10阳性率分别为4.49%、83.33%、3.21%,8.97%未能分型。其中15株CVA6分离毒株核苷酸和氨基酸序列同源性分别为91.85%~99.86%和85.81%~100.00%。系统进化树显示,15株CVA6位于同一分支,均属于D基因型中的D3基因亚型,与2018年黑龙江分离株MN845847、长春分离株MN845816、2015年上海分离株KX064297和2017年广东分离株MN845823进化关系较近,而与CVA6原型株Gdula进化距离较远。结论 2019-2020年深圳市大鹏新区HFMD病原CVA6型为绝对优势流行毒株,应加强对非EV71型、非CVA16型等其它肠道病毒监测。 Objective To understand the prevalence of coxsackievirus type A6(CVA6) which caused hand-foot-and-mouth disease(HFMD) in Dapeng New district of Shenzhen in 2019-2020, characterize the VP1 region of CVA6 isolates, and provide scientific evidence for the prevention and control of HFMD. Methods Anal swab samples from HFMD patients were collected, and the nucleic acids were detected by real-time fluorescent RT-PCR. The full-length CVA6 VP1 gene was amplified by RT-PCR and sequenced. The phylogenetic characteristics of the genes were analyzed at the molecular level using bioinformatics software.Results Among 156 enterovirus positive samples, the positive rates of CVA16, CVA6 and CVA10 were 4.49%, 83.33% and 3.21%, respectively, 8.97% could not be identified. The sequences of VP1 gene showed that the homologies of nucleotide and amino acid among the 15 isolates were 91.85%-99.86% and 85.81%-100.00%, respectively. The phylogenetic tree showed that 15 isolates of CVA6 were located in the same branch, belonging to the D3 gene subtype of the D genotype, which were closely related to the Heilongjiang isolates MN845847 and Changchun isolates MN845816 in 2018, Shanghai isolates KX064297 in 2015 and Guangdong isolates MN845823 in 2017, but far from the original CVA6 isolates Gdula. Conclusion CVA6 is the dominant isolate of HFMD in Dapeng New district of Shenzhen in 2019-2020. The surveillance of non-EV71, non-CVA16 and other enteroviruses should be strengthened.
