茯苓-泽泻药对配伍研究及其标准煎液对肾病模型大鼠的影响 被引量:12

Compatibility Research of Drug Pair Poria-Alismatis Rhizoma and its Effects on Nephropathy Model Rats

摘要 目的:根据古代医典茯苓泽泻药对经方配伍规律开展肾病综合征药效筛选,为相关病症的中医临床方案和新药开发提供理论和实验参考。方法:查阅同时含有茯苓和泽泻的祛湿类经方信息,量化表达后分析“茯苓-泽泻”药对配伍规律。按《医疗机构中药煎药室管理规范》制备其经典比例标准煎液。将不同组别大鼠分别连续灌胃21 d后,采用代谢笼测定各组大鼠24 h尿量,采用酶标仪和全自动生化分析仪测定24 h尿蛋白(UP)、血清白蛋白(Alb)、三酰甘油(TG)、总胆固醇(TC)、血尿素氮(BUN)和血清肌酐(SCr)指标,采用苏木精-伊红(HE)染色观察肾脏组织病理变化。结果:茯苓和泽泻在祛湿类经典方剂中配伍出现频次最高比例为1∶1,3∶5,2∶1;与模型对照组比较,给药3周后盐酸贝那普利组和不同配伍药对组24 h尿蛋白量均显著降低,血清Alb水平明显升高(P<0.01);不同药对组均有显著降TG作用,且以药对1∶1和3∶5组为最优;1∶1给药组较其他配伍组更显著降低SCr和BUN(P<0.05),并对肾组织病理损伤有一定的修复作用。结论:茯苓-泽泻药对配伍各给药组均能够显著减少多柔比星肾病大鼠24 h尿蛋白量,升高血清Alb水平,降低TC、TG水平,改善BUN、SCr,以茯苓-泽泻1∶1混煎为佳。 Objective: To perform the effect screening for nephrotic syndrome according to the compatibility law of drug pair Poria-Alismatis Rhizoma classical prescription in ancient medical canon, and provide theoretical and experimental reference for TCM clinical plan and new drug development of related diseases.Methods: The compatibility law of drug pair Poria-Alismatis Rhizoma was analyzed by quantitative expression of prescription information containing Poria and Alismatis Rhizoma. The standard decoction of the drug pair was prepared according to Management Specification of TCM Decoction Room in Medical Institutions. After continuous intragastric administration for 21 days, the 24 h urine volume of rats in different groups was measured by metabolic cage, and 24 h urine protein(UP), serum albumin(ALB), triacylglycerin(TG), cholesterol(CH), blood urea nitrogen(BUN) and serum creatinine(SCr) were measured by an enzyme plate analyzer and an automatic biochemical analyzer. The pathological changes of renal tissues were observed by hematoxylin-eosin(HE) staining. Results: The frequency of compatibility of Poria and Alismatis Rhizoma was the highest in dampness-removing classical prescriptions, and the commonly used proportions were 1∶1, 3∶5 and 2∶1. Compared with the model control group, after 3 weeks of administration, 24 h urinary protein level in benazepril hydrochloride group and different combination groups was significantly decreased, and serum Alb level was significantly increased(P<0.01). Lipid metabolism examination showed that different compatibility groups had significant TG lowering effect, and the optimal drug pairs were with the ratios of 1∶1 and 3∶5. Renal function test showed that 1∶1 administration group significantly reduced SCr and BUN indexes compared with the other groups(P<0.05), and had a certain repair effect on renal tissue pathological injury.Conclusion: Each administration group can significantly reduce 24 h urinary protein in rats with adriamycin nephropathy, increase serum Alb level, reduce CH and TG hyperlipidemia levels, improve BUN and SCr indexes of renal function, and the best drug pair is Poria-Alismatis Rhizoma at the ratio of 1∶1.
