摘要
目的:研究罗沙司他、多糖铁复合物联合治疗肾性贫血[慢性肾脏病(CKD)3~4期]的临床效果。方法:回顾性分析2020年1月-2021年12月九江市中医医院肾内科肾性贫血患者100例的临床资料,按治疗方法分为试验组、对照组,各50例。对照组患者接受人促红素注射液、右旋糖酐铁常规治疗,试验组患者在对照组基础上加用罗沙司他、多糖铁复合物治疗。分析两组患者的临床症状积分、贫血指标、微炎症因子、血清铁参数、临床疗效、不良反应发生情况。结果:两组患者治疗后的乏力、头晕、心力衰竭、心绞痛积分均低于治疗前(P<0.05);治疗后,试验组患者的乏力、头晕、心力衰竭、心绞痛积分均低于对照组(P<0.05)。两组患者治疗后的Hb、Hct均高于治疗前,iPTH、CRP均低于治疗前(P<0.05);治疗后,试验组患者的Hb、Hct均高于对照组,iPTH、CRP水平均低于对照组(P<0.05)。两组患者治疗后的血清转铁蛋白、铁蛋白、总铁结合力、转铁蛋白饱和度均高于治疗前(P<0.05);治疗后,试验组患者的上述指标均高于对照组(P<0.05)。试验组患者的总有效率高于对照组(χ^(2)=8.274,P=0.004)。试验组患者的不良反应发生率低于对照组(χ^(2)=5.316,P=0.021)。结论:罗沙司他、多糖铁复合物联合治疗肾性贫血(CKD 3~4期)的临床效果良好,能有效减轻患者临床症状和炎症反应,改善患者贫血状况和铁代谢,降低患者不良反应的发生。
Objective:To study the clinical effect of Roxadustat and Iron Polysaccharide Complex in the treatment of renal anemia(CKD stage 3-4).Method:The clinical data of 100 patients with renal anemia in Department of Nephrology,Jiujiang Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were retrospectively analyzed,according to the treatment method,they were divided into experimental group and control group,with 50 patients in each group.The patients in the control group received routine treatment with Recombinant Human Erythropoietin Injection and Iron Dextran,while the patients in the experimental group were treated with Roxadustat and Iron Polysaccharide Complex on the basis of the control group.The clinical symptom scores,anemia indicators,micro inflammatory factors,serum iron parameters,clinical efficacy and adverse reactions of the two groups were analyzed.Result:The scores of fatigue,dizziness,heart failure and angina pectoris in the two groups after treatment were lower than those before treatment(P<0.05);after treatment,the scores of fatigue,dizziness,heart failure and angina pectoris in the experimental group were lower than those in the control group(P<0.05).The Hb and Hct of the two groups after treatment were higher than those before treatment,and the iPTH and CRP were lower than those before treatment(P<0.05);after treatment,the levels of Hb and Hct in the experimental group were higher than those in the control group,and the levels of iPTH and CRP were lower than those in the control group(P<0.05).The serum transferrin,ferritin,total iron binding capacity and transferrin saturation of the two groups after treatment were higher than those before treatment(P<0.05);after treatment,the above indexes in the experimental group were higher than those in the control group(P<0.05).The total effective rate of patients in the experimental group was higher than that in the control group(χ^(2)=8.274,P=0.004).The incidence of adverse reactions in the experimental group was lower than that in the control group(χ^(2)=5.316,P=0.021).Conclusion:Roxadustat combined with Iron Polysaccharide Complex has a good clinical effect in the treatment of renal anemia(CKD stage 3-4),which can effectively reduce clinical symptoms and inflammatory response,improve anemia and iron metabolism,and reduce the incidence of adverse reactions in patients.
