摘要
目的分析股骨转子下骨折内固定术后影响髋关节功能的危险因素。方法将89例股骨转子下骨折患者按照末次随访时Harris评分不同分为功能优良组(评分≥80分,67例)和功能欠佳组(评分<80分,22例)。对影响术后髋关节功能的性别、年龄、致伤原因、ASA分级、合并症、骨质疏松程度、骨折类型、骨折分型、伤后至手术时间、内固定方式、手术时间、骨折复位质量、术后并发症发生情况、延迟负重情况等因素进行单因素分析。筛选单因素分析中有统计学意义的数据进行多因素logistic回归分析。结果性别、致伤原因、合并症、骨折分型、伤后至手术时间、手术时间、术后并发症发生情况两组比较差异均无统计学意义(P>0.05);年龄、ASA分级、骨质疏松程度、骨折类型、固定方式、骨折复位质量、延迟负重情况两组比较差异均有统计学意义(P<0.05)。多因素logistic回归分析显示:年龄、骨折类型、固定方式和延迟负重是影响股骨转子下骨折术后髋关节功能的独立危险因素(P<0.05)。结论在股骨转子下骨折内固定手术中,低龄、稳定性骨折、髓内固定和按期负重锻炼可获得更优的术后髋关节功能。
Objective To analyze the risk factors of hip joint function affecting hip joint function afteRfemoral subtrochanteric fracture surgery.Methods According to the different Harris scores at last follow-up,89 patients with femoral subtrochanteric fractures were divided into well-functioning group(scoring≥80 points,67 cases)and poor-functioning group(scoring<80 points,22 cases).The univariate analysis was performed on the factors affecting postoperative hip function,including gender,age,cause of injury,ASA classification,comorbidities,degree of osteoporosis,fracture type,fracture classification,time from injury to operation,internal fixation method,operation time,the quality of fracture reduction,postoperative complications,and delayed weight-bearing.The statistically significant data in univariate analysis were screened foRmultivariate logistic regression analysis.Results There were no statistical differences in gender,cause of injury,comorbidities,fracture classification,time from injury to operation,operation time,and postoperative complications between the two groups(P>0.05).There were statistically significant differences between the two groups in terms of age,ASA classification,degree of osteoporosis,fracture type,fixation method,fracture reduction quality and delayed weight-bearing(P<0.05).Multivariate logistic regression analysis showed that age,fracture type,fixation method and delayed weight-bearing were independent risk factors affecting hip function afteRsubtrochanteric fracture surgery(P<0.05).Conclusions In the internal fixation of femoral subtrochanteric fractures,young age,stable fractures,intramedullary fixation and regulaRweight-bearing exercises can achieve betteRpostoperative hip joint function.
