摘要
目的探讨产后盆底神经肌肉电刺激生物反馈联合凯格尔(Kegel)运动对盆底功能障碍(PFD)患者病情转归的影响。方法选择2019年8月至2020年12月肇庆市高要区人民医院收治的120例产后早期盆底功能障碍患者为研究对象,根据随机数表法分为观察组和对照组各60例。对照组实施Kegel运动训练,观察组实施盆底神经肌肉电刺激生物反馈联合Kegel运动训练。比较两组患者治疗后6个月的疗效、治疗前及治疗后6个月的肌纤维肌力评分、盆底功能障碍评分(PFDI-20)、盆底功能影响评分(PFIQ-7)、尿动力学[排尿时间(VT)、最大尿道压(MUP)、最大尿流率(Qmax)]指标,同时比较两组患者的并发症发生情况。结果治疗后6个月,观察组患者的总有效率为98.33%,明显高于对照组的80.00%,差异有统计学意义(P<0.05);治疗后6个月,观察组患者的Ⅰ类、Ⅱ类肌纤维肌力评分分别为(10.33±0.29)分、(12.04±0.46)分,明显高于对照组的(8.56±0.28)分、(10.01±0.47)分,PFDI-20、PFIQ-7评分、VT分别为(10.94±2.06)分、(10.12±2.04)分、(32.44±2.53)s,明显低于对照组的(18.05±3.77)分、(19.81±3.05)分、(37.01±2.49)s,MUP、Qmax分别为(86.02±4.35)cmH_(2)O、(27.30±5.11)mL/s,明显高于对照组的(79.17±4.08)cmH_(2)O、(23.27±5.45)mL/s,差异均有统计学意义(P<0.05);观察组并发症发生率为5.00%,明显低于对照组的26.67%,差异有统计学意义(P<0.05)。结论盆底神经肌肉电刺激生物反馈联合Kegel运动训练应用于产后早期盆底功能障碍能有效改善患者的盆底肌力和尿动力学指标,提高生活质量,临床应用效果显著,并发症低,值得推广使用。
Objective To explore the effect of postpartum pelvic floor neuromuscular electrical stimulation bio-feedback combined with Kegel exercise on the prognosis of patients with pelvic floor dysfunction(PFD).Methods A total of 120 patients with early postpartum pelvic floor dysfunction treated in Zhaoqing Gaoyao District People's Hospi-tal from August 2019 to December 2020 were selected as the research objects.They were randomly divided into an ob-servation group and a control group,with 60 patients in each group.The control group received Kegel exercise training,and the observation group received pelvic floor neuromuscular electrical stimulation biofeedback combined with Kegel exercise training.The curative effect at 6 months after treatment,muscle fiber strength score,pelvic floor dysfunction score(PFDI-20),pelvic floor function impact score(PFIQ-7),urodynamics[urination time(VT),maximum urethral pressure(MUP),maximum urinary flow rate(Qmax)]before treatment and at 6 months after treatment,and the incidence of complications were compared between the two groups.Results Six months after treatment,the total effective rate of the observation group was 98.33%,significantly higher than 80.00%of the control group(P<0.05).Six months after treat-ment,the muscle strength scores of classⅠand classⅡmuscle fibers in the observation group were(10.33±0.29)points and(12.04±0.46)points,which were significantly higher than(8.56±0.28)points and(10.01±0.47)points in the con-trol group;PFDI-20,PFIQ-7 scores,VT were(10.94±2.06)points,(10.12±2.04)points,(32.44±2.53)s,significantly lower than(18.05±3.77)points,(19.81±3.05)points,(37.01±2.49)s in the control group;MUP and Qmax were(86.02±4.35)cmH_(2)O,(27.30±5.11)mL/s,which were significantly higher than(79.17±4.08)cmH_(2)O,(23.27±5.45)mL/s in the control group;the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 5.00%,which was significantly lower than 26.67%in the control group(P<0.05).Conclusion Pelvic floor neuromuscular electrical stimulation biofeedback combined with Kegel exercise training in the treatment of early post-partum pelvic floor dysfunction can effectively improve the pelvic floor muscle strength and urodynamic indexes of pa-tients,improve the quality of life,and brings significant clinical effect and low incidence of complications,which is wor-thy of popularization.
