摘要
目的探讨PBL结合迷你临床演练评估(mini-clinical evaluation exercise,Mini-CEX)在神经病学临床教学中的应用。方法对2018年3月—2020年3月进入上海长海医院脑血管病中心进行临床培训的共26名研究生以及由我科8名教师组成的师资团队进行研究,比较PBL课程前后研究生的Mini-CEX考核评分,结合问卷调查评价PBL教学结合Mini-CEX在神经病学专科研究生临床教学应用中的效果。结果研究生临床能力各项目评分均值除病史询问项目以外,均较前提升,在人文关怀、临床判断、卫教咨询、组织效能、整体表现5个方面,研究生的考核成绩较课程前成绩差异有统计学意义(P<0.05)。问卷调查结果显示,80.77%的研究生对考试形式感到赞同,没有因为老师在场评估而影响能力的发挥。92.31%的研究生觉得考试时间充足,获得了较好的锻炼机会,能够充分展现自己的临床能力,92%以上的研究生对考核中的反馈内容,评估方式感到赞同,觉得考核中反馈的内容“使自己明白了自身的优缺点”,对自己的学习有很大的帮助。88%以上的研究生认为PBL课程结合Mini-CEX考核提高了自身的各项临床能力。结论PBL教学结合Mini-CEX考核能全面提高研究生的临床能力,促进教学方法的改进。
Objective To explore the application of Mini-CEX combined with problem-based learning(PBL)in clinical teaching of neurology.Methods A total of 26 graduate students and a teaching team composed of 8 teachers from March 2018 to March 2020 were enrolled in the department of stroke center of Changhai Hospital for clinical training.The Mini-CEX assessment scores of graduate students before and after PBL course were compared.Combined with questionnaire survey,the effect of PBL teaching combined with Mini-CEX in clinical teaching of neurology graduate students was evaluated.Results The average scores of all items of Postgraduates'clinical ability,except the medical history inquiry items,were higher than the previous ones.In the five aspects of humanistic care,clinical judgment,health education consultation,organizational efficiency and overall performance,the examination results of postgraduates were significantly different from those before the course(P<0.05).The results of the questionnaire showed that 80.77%of the graduate students agreed with the examination form,and did not affect their ability because of the presence of the teacher.92.31%of the graduate students felt that the examination time was sufficient,they obtained better exercise opportunities,and they were able to fully demonstrate their clinical ability.More than 92%of the graduate students agreed with the feedback content and evaluation method in the assessment,and felt that the feedback content in the assessment"made themselves understand their own advantages and disadvantages",which was of great help to their study.More than 88%of graduate students believe that PBL courses combined with Mini-CEX examination had improved their clinical abilities.Conclusion PBL teaching combined with Mini-CEX examination can comprehensively improve the clinical ability of postgraduates and elevate the improvement of teaching methods.
