血清降钙素原、D-二聚体及脑钠肽在急性心肌梗死患者经皮冠状动脉介入术术后心功能及预后中的应用价值 被引量:3

摘要 目的 探讨血清降钙素原(PCT)、D-二聚体(DD)及脑钠肽(BNP)在急性心肌梗死患者(AMI)经皮冠状动脉介入术(PCI)术后心功能及预后中的应用价值。方法 选取2017年11月至2019年11月本院收治的100例AMI患者作为研究对象,根据患者术后是否出现不良心血管事件(MACE)分为MACE组(n=36)和非MACE组(n=54)。比较两组血液指标、超声检查指标和临床疗效。结果 MACE组PCT、DD及BNP水平均明显高于非MACE组(P<0.05)。MACE组左室舒张末期内经(LVEDD)和左房内径(LAD)值均明显大于非MACE组,左室短轴缩短率(FS)和左室射血分数(LVEF)值均明显低于非MACE组,差异均有统计学意义(P<0.05)。ROC曲线显示,PCT预测AMI患者MACE发生率的AUC=0.945,DD的预测AUC=0.795,BNP的预测AUC=0.748。非MACE组手术总有效率明显高于MACE组,差异有统计学意义(P<0.05)。结论 血清PCT、DD及BNP水平可反映急性心肌梗死患者PCI术后的心功能状态,为AMI患者预后评估提供参考。 Objective To investigate the application value of serum procalcitonin(PCT), D-dimer(DD) and brain natriuretic peptide(BNP) in patients with acute myocardial infarction(AMI) after percutaneous coronary intervention(PCI) on cardiac function and prognosis. Methods A total of100 AMI patients admitted to our hospital from November 2017 to November 2019 were selected as the research subjects, and they were divided into MACE group(n=36) and non-MACE group(n=54) according to whether there was adverse cardiovascular event(MACE) after operation. The blood indexes, ultrasonography indexes and clinical efficacy were compared between the two groups. Results The levels of PCT, DD and BNP in the MACE group were significantly higher than those in the non-MACE group(P<0.05). The left ventricular end-diastolic diameter(LVEDD) and leftatrial diameter(LAD) values in the MACE group were significantly larger than those in the non-MACE group, and the fraction shortening(FS) and left ventricular ejection fraction(LVEF) values were significantly lower than those in the non-MACE group, and the difference was statistically significant(P<0.05). The ROC curve showed that PCT predicted the incidence of MACE in AMI patients was AUC=0.945, the predicted probability of DD was AUC=0.795, and the predicted probability of BNP was AUC=0.748. The total effective rate of operation in the non-MACE group was significantly higher than that in the MACE group, and the difference was statistically significant(P<0.05). Conclusion Serum PCT, DD and BNP levels can reflect the cardiac function status of patients with acute myocardial infarction after PCI, and provide reference value for the prognosis evaluation of patients with AMI.
