摘要
喻清和主任医师认为支气管扩张症的病位在肺,涉及脾肾,与胃肠亦相关,痰、热、瘀、虚为其病机要点,且常兼夹并存。辨治支气管扩张症应扶正祛邪并举,治疗采用清热化痰、祛瘀通腑之法以治标,调补肺脾肾三脏以治本。化痰为要,可采用苇茎汤(由苇茎、冬瓜仁、薏苡仁、桃仁组成);化瘀止血,气血同治;补肺健脾,温肾固本,兼顾肺脾肾三脏虚损。喻清和主任同时注重支气管扩张症患者稳定期的调护,嘱咐患者注意饮食与起居的调节,适当运动,作息规律,提高机体抵抗力。
The chief physician YU Qing-He believes that the disease-location of bronchiectasis is in the lung,involves the spleen and kidney,and is also related to the stomach and intestines.The pathogenesis of bronchiectasis is characterized by pathological factors of phlegm,heat,stasis and deficiency,and the mixture and coexisting of the pathological factors.The treatment of bronchiectasis should follow the principles of simultaneous use of supporting healthy qi and eliminating pathogens,i.e.,using the method of clearing heat and resolving phlegm,and eliminating stasis and unblocking bowels to treat the symptoms,and tonifying the lung,spleen and kidney to treat the root cause.Resolving phlegm is the key to the treatment of bronchiectasis,and Weijing Decoction(composed of Caulis Phragmitis,Semen Benincasae,Semen Coicis and Semen Persicae)can be adopted.Resolving stasis and stopping bleeding can achieve the efficacy of treating qi and blood simultaneously,and the therapy of tonifying lung and invigorating spleen,warming kidney and consolidating vital base can take into account the deficiency of lung,spleen and kidney.YU Qing-He suggests that we should focus on the care of bronchiectasis patients during the stable period,i.e.,paying attention to the management of diet and daily living,doing appropriate exercises,and keeping regular living style to improve the immunity of human body.
