摘要
目的基于隐结构结合关联规则探讨中医药治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭(RF)的组方规律。方法检索中国知识资源总库(CNKI)、中国学术期刊数据库(Wanfang Data)、中文科技期刊数据库(VIP Database)、中国生物医学文献数据库(CBM)、PubMed、Cochrane Library、Embase、Web of science等数据库自建库起至2021年1月期间收录的涉及中医药治疗COPD合并RF的临床观察或随机对照试验文献。按照预先设定的纳入、排除标准进行文献筛选,提取相关信息并建立数据库。分别采用Lantern 5.0、SPSS Molder 18.0及Cytoscape 3.7.2软件,对频率≥3%的中药进行隐结构及关联规则分析,并结合方药频次、频率分析中药治疗COPD合并RF的组方规律。结果共纳入184篇文献,涉及中药145味、方剂59首。中药频率从高到低依次为茯苓、甘草、陈皮、瓜蒌、苦杏仁等,以化痰止咳平喘药、补益药、理气药为主,药性偏温、寒、平,药味偏甘、辛、苦,主归肺、脾、胃、心经。方剂频率从高到低依次为清肺化痰汤、燥湿化痰丸、人参补肺汤、二陈汤、血府逐瘀汤、健脾益气汤等,以祛痰剂、补益剂、祛湿剂为主。隐结构分析得到8个中医常见证候。关联规则分析筛选得到44条强关联规则。结论中医药治疗COPD合并RF以健脾化痰药为主,结合辨证选用补气养阴、止咳平喘、活血化瘀类中药遣药组方,以补肺健脾益肾治其本,止咳化痰活血治其标。
Objective To investigate the prescription regularity of traditional Chinese medicine(TCM)for the treatment of chronic obstructive pulmonary disease(COPD)complicated with respiratory failure(RF)based on latent structure combined with association rules.Methods Computer retrieval of CNKI,Wanfang Data,VIP Database,CBM,PubMed,Cochrane Library,Embase,Web of science was performed for collecting the clinical observation or randomized controlled trials involving TCM treatment of COPD complicated with RF from its establishment to January 2021.The literature was screened according to the pre-defined inclusion and exclusion criteria,and relevant information was extracted and the database was established.Lantern 5.0,SPSS Molder 18.0 and Cytoscape 3.7.2 software were used to analyze the latent structure and association rules of TCM with a frequency of≥3%,and to analyze the prescription regularity of TCM treatment of COPD complicated with RF by combining the frequency of prescriptions.Results A total of 184 literature were included,involving 145 Chinese herbal medicines and 59 prescriptions.The Chinese herbal medicines in descending order of frequency are Poria,Radix Glycyrrhizae,Pericarpium Citri Reticulatae,Fructus Trichosanthis,and Semen Armeniacae Amarum,and the herbs are mainly functioned as resolving phlegm,easing coughing and relieving asthma,tonifying and replenishing,and regulating qi,while the herbs are tend to be warm,cold and mild in nature,and sweet,pungent and bitter in flavour,and the tropism mainly belongs to the lung,spleen,stomach and heart meridian.The prescriptions in descending order of frequency were Qingfei Huatan Decoction,Zaoshi Huatan Pills,Renshen Bufei Decoction,Erchen Decoction,Xuefu Zhuyu Decoction,and Jianpi Yiqi Decoction,which are mainly functioned as expectorants,tonifiers and dampness-removing agents.The latent structure analysis showed 8 common TCM syndromes,and the association rule analysis was screened to obtain 44 strong association rules.Conclusion TCM treatment of COPD complicated with RF is based on spleen-strengthening and phlegm-resolving,combined with the syndrome differentiation to choose the composition of prescriptions with functions of replenishing qi and nourishing yin,easing cough and relieving asthma,activating blood and resolving stasis,in order to replenish lung,invigorate spleen and nourish kidney to treat its root cause,and ease cough and dissolve phlegm and activate blood to treat its symptoms.
