摘要
【目的】探讨疏风利咽汤治疗顽固性喉源性咳嗽的疗效及对气道阻力的影响。【方法】选取2017年1月至2020年6月在陕西省中医医院耳鼻喉科诊治的102例顽固性喉源性咳嗽患者,按随机数字表法分为观察组和对照组,每组51例。对照组患者给予常规西药治疗,观察组患者在对照组基础上给予疏风利咽汤治疗。比较两组患者临床疗效及治疗前后中医证候评分、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、近咽部黏性阻力(R_(35))、中心气道黏性阻力(R_(20))、呼吸道总阻抗(Zrs)水平。【结果】观察组总有效率为92.16%,显著高于对照组的72.55%(P<0.05);两组治疗后各项中医证候评分显著低于治疗前,且观察组显著低于对照组(P<0.05);两组治疗后IgA、IgG、IgM显著高于治疗前,且观察组显著高于对照组(P<0.05);两组治疗后R_(35)、R_(20)、Zrs显著低于治疗前,且观察组显著低于对照组(P<0.05)。【结论】疏风利咽汤治疗顽固性喉源性咳嗽的疗效较好,对患者的临床症状、免疫功能、气道阻力均有一定改善作用,值得临床推广应用。
【Objective】To explore the effect of Shufeng Liyan Decoction in the treatment of intractable laryngeal cough and its effect on airway resistance.【Methods】A total of 102 patients with intractable laryngeal cough diagnosed and treated in the Department of Otorhinolaryngology of the Shaanxi Traditional Chinese Medicine Hospital from January 2017 to June 2020 were randomly divided into observation group and control group,with 51 cases in each group.The patients in the control group were treated with conventional western medicine,and the patients in the observation group were treated with Shufeng Liyan Decoction on the basis of the control group.The clinical efficacy,TCM syndrome score,immunoglobulin A(IGA),immunoglobulin G(IgG),immunoglobulin M(IgM),proximal pharyngeal viscous resistance(R_(35)),central airway viscous resistance(R_(20)),and total respiratory tract impedance(Zrs)levels were compared between the two groups before and after treatment.【Results】The total effective rate of the observation group was 92.16%,significantly higher than 72.55% of the control group(P<0.05);The TCM syndrome scores of the two groups after treatment were significantly lower than those before treatment,and the observation group was significantly lower than that of the control group(P<0.05);IgA,IgG and IgM in the two groups after treatment were significantly higher than those before treatment,and those in the observation group were significantly higher than those in the control group(P<0.05);R_(35),R_(20) and Zrs in the two groups after treatment were significantly lower than those before treatment,and those in the observation group were significantly lower than those in the control group(P<0.05).【Conclusion】The Shufeng Liyan decoction is effective in the treatment of intractable laryngeal cough.It can improve the clinical symptoms,immune function and airway resistance of patients,which is worthy of clinical application.
