基于倾向性评分匹配分析复方丹参滴丸联合盐酸氟西汀胶囊治疗抑郁症的研究 被引量:3

摘要 目的基于倾向性评分匹配分析复方丹参滴丸联合盐酸氟西汀胶囊治疗抑郁症的临床效果。方法回顾性分析,收集2018年1月至2021年5月接受盐酸氟西汀胶囊治疗的95例抑郁症患者资料纳入单药组;收集同期接受复方丹参滴丸联合盐酸氟西汀胶囊治疗的88例抑郁症患者资料,纳入联合组;使用倾向性评分匹配分析患者基线资料,采用近邻法将联合组与单药组进行1∶1匹配,共61对匹配成功;分析比较匹配后2组患者治疗前、治疗12周抑郁程度[依据汉密尔顿抑郁量表(HAMD)评估]、血清炎性因子水平[白细胞介素(IL)-6、IL-1β、肿瘤坏死因子-α(TNF-α)];统计2组患者药物不良反应发生情况。结果倾向性评分匹配前,联合组与单药组患者年龄、性别、合并症、共患病类型、共患病病程、抑郁症病程、抑郁症程度比较,差异有统计学意义(P<0.05);经倾向性评分匹配共匹配成功61对,匹配后联合组与单药组患者年龄、性别、合并症、共患病类型、共患病病程、抑郁症病程、抑郁症程度比较,差异无统计学意义(P>0.05);匹配后2组患者治疗12周HAMD评分低于治疗前,且联合组低于单药组,差异有统计学意义(P<0.05);匹配后2组患者治疗12周IL-6、IL-1β、TNF-α水平低于治疗前,且联合组低于单药组,差异有统计学意义(P<0.05);匹配后2组药物不良反应总发生率比较差异无统计学意义(P>0.05)。结论复方丹参滴丸联合盐酸氟西汀胶囊治疗抑郁症,可改抑制机体炎性介质释放,减轻患者抑郁程度,临床疗效显著,且安全性好。 Objective To investigate the clinical effects of compound Danshen dropping pills combined with fluoxetine hydrochloride capsules in treatment of depression evaluated by propensity score matching.Methods A total of 95 patients with depression who were treated by fluoxetine hydrochloride capsules in our hospital from January 2018 to May 2021 were enrolled as single drug group,at the same time,the other 88 patients with depression who were treated by ompound Danshen dropping pills combined with fluoxetine hydrochloride capsules were enrolled as combination treatment group.The baseline data of patients were analyzed by propensity score matching,and the degree of depression[assessed by Hamilton Depression Scale(HAMD)]and the serum levels of inflammatory factors[interleukin(IL)-6,IL-1βand tumor necrosis factor-α(TNF-α)]before treatment and at 12 weeks after treatment as well as the incidence of drug adverse reactions were observed and compared between the two groups.Results Before propensity score matching,there were significant differences in patient's age and gender,complications,type of comorbidity,course of comorbidity,course of depression and degree of depression of patients between the two groups(P<0.05).After propensity score matching,61 pairs were successfully matched,there were no significant differences in the indexes mentioned above between the two groups(P>0.05).After matching,the HAMD scores in both groups after 12-week treatment were significantly decreased,moreover,which in combination treatment group were significantly lower than those in single drug group(P<0.05).After matching,the serum levels of IL-6,IL-1βand TNF-αin both groups after 12-week treatment were significantly decreased,moreover,which in combination treatment group were significantly lower than those in single drug group(P<0.05).However there was no significant difference in the total incidence of adverse drug reactions between the two groups(P>0.05).Conclusion The compound Danshen dropping pills combined with fluoxetine hydrochloride capsules in treatment of depression can inhibit the release of inflammatory mediators and relieve the depression degree of patients,with obvious clinical efficacy and better safety.
