摘要
厥证属于六经病,厥热胜复是厥证的一种急性临床表现,各经病均可发展至此阶段,预后视正邪斗争情况而定。由各经病而来的厥证有其本经的特点,热厥主要是因邪热深伏于内,阳气被遏,不能外达四肢所致,其外在表现似寒,实为真热,包含太阳厥证、少阳厥证、阳明厥证,方用麻黄杏仁甘草石膏汤、小柴胡汤、白虎汤、承气汤等。寒厥为阴寒内盛,阳气哀微,机体失其温养所致,是一种虚寒性疾病,包含太阴厥证、少阴厥证、厥阴厥证,治疗上以温阳散寒,回阳救逆为主,方用理中丸、附子汤、四逆汤、白通汤等。除六经厥证,还包括各类杂厥,病机为阳气不能敷布四末,包括气厥、血厥、水厥、蛔厥、痰食厥、脏厥、浊厥、关元冷厥等。各类厥证虽然形成病因不尽相同,然总不离表里阴阳寒热虚实,在临床中见外象厥逆,应注意审证求因,识其内在病机而治,并及时在经截断,防厥逆进展。
Syncope belong to diseases of the six meridians,and the struggle of syncope fever is an acute clinical manifestation of syncope.All meridians'diseases can develop to this stage,and the prognosis depends on the fight between healthy and pathogenic qi.Syncope from various meridians has the characteristics of its own meridian.Heat syncope is mainly caused by the deep latent pathogenic heat,and the yang qi cannot reach the limbs.Its appearance looks like cold,but it is really hot,including syncope of greater yang,syncope of lesser yang,syncope of yang brightness.Ephedra and Apricot Kernel and Gypsum and Licorice Decoction,Minor Bupleurum Decoction,White Tiger Decoction,Purgative Decoction,etc.can be adopted.Cold syncope is caused by excessive internal yin cold,deficient yang qi,and loss of the body nourishment.It is a deficiency cold disease,including syncope of greater yin,lesser yin disease,and syncope of reverting yin.The treatment mainly focuses on warming yang and dispersing cold,restoring yang from collapse,so Center-Regulating Pill,Aconite Decoction,Frigid Extremities Decoction,Scallion Yang-Freeing Decoction,etc.can be used.In addition to syncope of the six meridians,there are all types of miscellaneous syncope,including qi syncope,blood syncope,water syncope,ascariasis syncope,phlegm syncope,visceral syncope,turbid syncope,cold syncope at Guanyuan(BL 26),etc.The pathogenesis is that yang qi cannot reach the ends of four limbs.Although the causes of various types of syncope are different,they are always related to the deficiency and excess and cold and heat in the exterior-interior yin and yang.In clinical practice,for any external symptoms of syncope,it is necessary to find out the syndrome and cause,identify the internal pathogene-sis,prevent the progress in time in the meridian.
