低剂量螺旋CT联合血浆miR-200b、miR-200c检查鉴别诊断良恶性肺结节的价值 被引量:3

Value of low-dose spiral CT combined with plasma miR-200b and miR-200c in differentiating diagnosis of benign and malignant pulmonary nodules

摘要 目的探讨低剂量螺旋CT(low-dose CT,LDCT)联合血浆miR-200b、miR-200c检查鉴别诊断良恶性肺结节的临床应用价值。方法选取2017年1月至2020年12月在江西省安福县中医院低剂量螺旋CT初次筛查的113例Lung-RADS 4类肺结节患者为研究对象,后均经病理组织学确诊癌性结节78例(肺癌组)、良性结节35例(良性组),两组患者均采用实时荧光定量聚合酶链反应(qRT-PCR)检测血浆miR-200b、miR-200c相对表达量。结果①影像学显示癌性结节组磨玻璃样改变、分叶征、毛刺征、胸膜凹陷、血管集束征等发生率明显高于良性结节组(χ^(2)=34.541、20.378、14.113、11.249、27.371,P<0.001、<0.001、<0.001、0.001、<0.001),P<0.01。②癌性结节组患者血浆miR-200b、miR-200c相对表达量明显低于良性结节组(U=94.000、83.500,P=0.012、0.005)。③低剂量螺旋CT联合血浆miR-200b、miR-200c检查鉴别诊断恶性肺结节敏感性、准确性分别为93.59%、91.50%(χ^(2)=4.673、4.102,P=0.031、0.043),与各单项检查比较明显提高(P<0.05)。结论低剂量螺旋CT联合血浆miR-200b、miR-200c鉴别良恶性肺结节,有助于肺癌早期诊断。 Objective To explore the clinical value of low-dose spiral CT(LDCT)combined with plasma miR-200b and miR-200c in differential diagnosis of benign and malignant pulmonary nodules.Methods A total of 113 patients with Lung-RADS 4 pulmonary nodules who were initially screened by low-dose spiral CT in Anfu Hospital of Traditional Chinese Medicine of Jiangxi Province from January 2017 to December 2020 were selected as the research objects.After that,78 patients were diagnosed as cancerous nodules(Lung cancer group)and 35 patients were diagnosed as benign nodules(benign group)by hihistology.Quantitative real-time polymerase chain reaction(qRT-PCR)was used to detect the relative expression levels of miR-200b and miR200c in plasma of both groups.Results①Imaging showed that the incidence of ground-glass changes,lobular sign,burr sign,pleural indentation and vascular clustering in the lung cancer nodules group was significantly higher than that in the benign nodules group(χ^(2)=34.541,20.378,14.113,11.249,27.371,respectively,P<0.001,<0.001,<0.001,0.001,<0.001),P<0.01.②The relative expression levels of miR-200b and miR-200c in the cancer nodules group were significantly lower than those in the benign nodules group(U=94.000,83.500,P=0.012,0.005).③The sensitivity and accuracy of low-dose spiral CT combined with plasma miR-200b and miR-200c in differential diagnosis of malignant pulmonary nodules were 93.59%and 91.50%,respectively(χ^(2)=4.673,4.102,P=0.031,0.043),which were significantly higher than those of each single examination(P<0.05).Conclusion Low dose spiral CT combined with plasma miR-200b and miR-200c in differentiating benign and malignant pulmonary nodules is helpful for early diagnosis of lung cancer.
