摘要
目的:探讨宫腔镜子宫瘢痕憩室(CSD)切除联合腹腔镜子宫下段肌层折叠缝合修复治疗剖宫产术后CSD的临床效果。方法:选取2021年1月-2022年1月福建中医药大学附属厦门第三医院剖宫产术后68例CSD患者为研究对象。依据随机抽签法将其分为研究组和对照组,各34例。对照组给予宫腔镜引导下经腹腔镜切除子宫瘢痕后重新缝合修复。研究组给予宫腔镜CSD切除联合腹腔镜子宫下段肌层折叠缝合修复。比较两组围手术期指标、术前及术后3个月子宫下段肌层厚度、术前及术后生活质量。结果:两组手术时间、术后住院时间比较差异均无统计学意义(P>0.05),研究组术中出血量少于对照组(P<0.05)。术后3个月,研究组子宫下段肌层厚度大于对照组(P<0.05)。术后研究组躯体功能、心理活动、社会能力、物质生活状态评分均高于对照组(P<0.05)。结论:剖宫产术后CSD患者应用宫腔镜CSD切除联合腹腔镜子宫下段肌层折叠缝合修复治疗效果理想,有效减少术中出血量,改善患者子宫下段肌层厚度及生活质量,值得推广。
Objective:To investigate the clinical effect of hysteroscopic CSD resection combined with laparoscopic muscularis of lower segment of uterus fold suture repair in the treatment of CSD after cesarean section.Method:From January 2021 to January 2022,68 patients with CSD after cesarean section in Xiamen Third Hospital Affiliated to Fujian University of TCM were selected as the research objects.They were divided into study group and control group according to the random lottery method,with 34 cases in each group.The control group was given the hysteroscopic guided via laparoscopic resection of uterine scar and resuture repair.The study group was given the hysteroscopic CSD resection combined with laparoscopic muscularis of lower segment of uterus fold suture repair.The perioperative indexes,the thickness of the muscularis of lower segment of uterus before and 3 months after operation,and the quality of life before and after operation were compared between the two groups.Result:There were no significant differences in operation time and postoperative hospital stay between the two groups(P>0.05),and the amount of intraoperative bleeding in the study group was less than that in the control group(P<0.05).Three months after operation,the thickness of muscularis of lower segment of uterus in the study group was greater than that in the control group(P<0.05).After operation,the scores of physical function,psychological activity,social ability and material life state in the study group were higher than those in the control group(P<0.05).Conclusion:Hysteroscopic CSD resection combined with laparoscopic muscularis of lower segment of uterus fold suture repair in the application of CSD patients after cesarean section has an ideal treatment effect,which can effectively reduce the amount of intraoperative bleeding and improve the thickness of muscularis of lower segment of uterus and quality of life of patients,which is worthy of promotion.
