首发产后抑郁症患者脑功能网络间功能连接研究 被引量:5

Functional Connectivity Between Functional Brain Networks in Patients with First-Episode Postpartum Depression

摘要 目的:探讨首发产后抑郁症(PPD)患者静息态下脑功能网络间功能连接(FC)的变化特点及价值。方法:收集28名首发PPD患者(病例组)与28名健康产妇(对照组)进行颅脑静息态功能磁共振成像,使用独立成分脑网络模板作为脑部感兴趣区,进行感兴趣脑区之间功能连接分析,并比较2组各脑网络脑区间的差异。结果:与对照组相比,病例组额顶网络左外侧前额叶皮质与感觉运动网络各脑区(顶部t=-3.58、P=0.013,左侧t=-3.19、P=0.013,右侧t=-2.82、P=0.023,FDR校正) FC减弱,病例组额顶网络右外侧前额叶皮质与感觉运动网络左侧感觉运动区(t=-2.76,P=0.042,FDR校正) FC减弱;病例组默认网络内侧前额叶皮质与背侧注意网络右侧额叶视区(t=-3.04,P=0.041,FDR校正) FC减弱;病例组突显网络前扣带皮质与额顶网络左外侧前额叶皮质(t=-2.73,P=0.047,FDR校正)、默认网络内侧前额叶皮质(t=-2.72,P=0.047,FDR校正) FC减弱。结论:首发PPD患者可能存在额顶网络受损、默认网络活动改变、突显网络调节障碍,以及这3个神经认知网络之间存在交互作用异常。 Purpose: To investigate the characteristics and value of functional connections between functional brain networks in patients with first-episode postpartum depression(PPD) in resting state. Methods: Twenty-eight patients with first-episode PPD(case group) and twenty-eight healthy postpartum women(control group) were collected for cerebral resting state functional magnetic resonance imaging. The independent component brain network template was used as the brain area of interest, and the functional connectivity between the brain areas of interest was analyzed to compare the differences of functional connectivity between the two groups. Results:Compared with the control group, the functional connection between the left lateral prefrontal cortex of the frontoparietal network and the superior sensorimotor area(t=-3.58, P=0.013, FDR correction),the left sensorimotor area(t=-3.19, P=0.013, FDR correction) and the right sensorimotor area(t=-2.82, P=0.023, FDR correction) of the sensorimotor network decreased in the case group;the functional connection between the right lateral prefrontal cortex of the frontal parietal network and the left sensorimotor area of the sensorimotor network(t=-2.76, P=0.042,FDR correction) decreased in the case group;the functional connection between the medial prefrontal cortex of the default mode network and the right frontal visual area of the dorsal attention network(t=-3.04, P=0.041, FDR correction) decreased in the case group;the functional connection between the anterior cingulate cortex of salience network and the left lateral prefrontal cortex of the frontal parietal network(t=-2.73, P=0.047, FDR correction) and the medial prefrontal cortex of the default mode network(t=-2.72, P=0.047, FDR correction) decreased in the case group. Conclusion: Patients with first-episode PPD may have impaired frontoparietal network and altered default mode network activity, salience network regulation disorders, and abnormal interaction between these three neurocognitive networks.
