摘要
目的:探究移植肾功能延迟恢复(DGF)的超声造影(CEUS)特征。方法:回顾性分析2018年11月至2021年10月在我院行肾移植手术的86例患者,所有患者在移植术后3~5 d均接受CEUS检查,观察移植肾CEUS的增强模式,选择皮质和髓质作为感兴趣区(ROI)生成时间-强度曲线(TIC),比较DGF组与移植肾功能正常恢复(NGF)组的时间参数、强度参数和参数差值。结果:DGF组较NGF组移植肾内各级动脉阻力指数(RI)升高、叶间动脉收缩期峰值流速(PSV)降低,CEUS检查DGF组出现“脉冲式”增强的概率大于NGF组(P<0.001),DGF组ATTP大于NGF组(P<0.01),AUC低于NGF组(P<0.01),DGF组皮质与髓质的ATTP差值(ATTP)大于NGF组(P<0.05)、皮质AUC与髓质AUC的差值(AUC)小于NGF组(P<0.05)。结论:CEUS可观察移植肾的增强模式以及微循环灌注情况,对早期诊断DGF具有一定临床意义。
Objective:To investigate the contrast-enhanced ultrasound(CEUS) performance and its parameters of delayed graft function(DGF) of renal transplantation. Methods: There is a retrospective analysis of 86 cases after renal transplantation, all patients received contrast-enhanced ultrasound(CEUS) examination 3-5 days after transplantation. To observe the enhanced mode of CEUS in the transplanted kidney. The cortex and medulla of the transplanted kidney were selected as the regions of interest(ROI), time parameters, intensity parameters, and difference parameters of two groups were determined by time intensity curve(TIC).Results: RI of all arteries of kidney in DGF group was increased, and PSV of interlobar artery was decreased, the probability of “pulse” enhancement in CEUS examination of transplanted kidney in the DGF group was higher than that in the NGF group, the ATTP in DGF group was higher than that in NGF group, the AUC was lower than that in NGF group, ATTP、 AUCin DGF group was lower than that in NGF group.Conclusion:The enhancement mode and microcirculation perfusion of transplanted kidney with CEUS, which has certain clinical significance for the early diagnosis of DGF are observed.
