摘要
目的探讨早期俯卧位通气对主动脉夹层术后低氧血症患者氧合状况的改善效果及其对血流动力学稳定性的影响。方法选取本科室2021年1~10月期间收治的89例主动脉夹层术后低氧血症患者作为研究对象,根据随机数字表法分为对照组44例和观察组45例,干预16 h后,对照组行仰卧位通气,观察组给予早期俯卧位通气,对比两组患者干预前后的氧合状况[动脉血氧分压(PaO_(2))、经皮氧饱和度(SpO_(2))、氧合指数(OI)],血流动力学稳定性[平均动脉压(MAP)、心率(HR)],炎性反应水平[肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)]。结果两组患者治疗后,PaO_(2)、SpO_(2)、OI水平较治疗前升高,且观察组高于对照组(P<0.05);MAP较治疗前上升,HR较治疗前降低,组间差异无统计学意义(P>0.05);TNF-α、IL-6水平较治疗前下降,且观察组低于对照组(P<0.05)。结论早期俯卧位通气能有效改善低氧血症患者的氧合状况,保持血流动力学稳定,降低炎症反应,促进患者康复。
Objective To investigate the effect of early prone ventilation on the improvement of oxygenation status in patients with hypoxemia after aortic dissection and its influence on hemodynamic stability.MethodsA total of 89 patients with hypoxemia after aortic dissection admitted to our department from January 2021 to October 2021 were selected as the research subjects,and were divided into control group(n=44 cases)and observation group(n=45 cases)according to random number table method.The control group received supine ventilation for 16 h,and the observation group received early prone ventilation.Oxygenation status[arterial partial pressure of oxygen(PaO2),percutaneous oxygen saturation(SpO2),oxygenation index(OI)],hemodynamic stability[mean arterial pressure(MAP),heart rate(HR)],inflammatory response level[tumor necrosis factorα(TNF-α),interleukin-6(IL-6)]were compared between two groups before and after intervention.Results After treatment,the levels of PaO_(2),SpO_(2) and OI in two groups were higher than before,and the observation group was higher than the control group(P<0.05).MAP increased and HR decreased,with no significant difference between groups(P>0.05).The levels of TNF-αand IL-6 in the observation group were lower than those in the control group(P<0.05).Conclusion Early prone ventilation can effectively improve the oxygenation status of patients with hypoxemia,maintain hemodynamic stability,reduce inflammatory response,and promote the recovery of patients.
