摘要
目的分析血液初筛时TP和HbsAg漏检的影响因素,为减少血液浪费和保障血液安全提供依据.方法选取南宁中心血站2021年7月-11月献血者的血液标本60521份,使用酶联免疫法进行TP和HbsAg检测,检测结果呈双反应性的标本按照血液初筛要求进行胶体金法复测,对比初筛环节影响因素,探索血液初筛TP和HbsAg漏检原因及改善措施.结果60521份血液标本经酶联免疫法检测后TP和HbsAg反应性标本共112份,占所有经血传播疾病检测项目237份不合格标本的47.26%.其中HbsAg反应性标本60例、TP反应性标本52例.反应性标本用胶体金法检测HbsAg阳性27例(45%)、阴性33例(55%);TP阳性37例(71.15%)、阴性15例(28.85%).不同S/CO值金标法检测阳性结果比例差别有统计学意义(P<0.05).外采和血屋不同采血人数TP和HbsAg阳性占比差别有统计学意义(P<0.05).血屋采血人数16人以上TP和HbsAg漏检比例为32.81%,外采人数超过100人时TP和HbsAg漏检比例为45.31%.结论胶体金法检测TP和HbsAg存在一定的漏检比例,做好初筛环节的关键点控制,加强工作人员职业技能培训,保障献血场所人员配备可以进一步降低TP和HbsAg漏检率.
Objective To analyze the influencing factors of missing delection of TP and HBsAg in primary blood screening,so as to provide basis for reducing blood waste and ensuring blood safety.Methods 60521 blood samples from blood donors in our station from July to November 2021 were selected.TP and HBsAg were detected by enzyme-linked immunosorbent assay.The samples with double reactivity were retested by colloidal gold method according to the requirements of primary blood screening.The influencing factors of primary screening were compared to explore the causes of missed detection of TP and HBsAg and improvement measures.Results After 60521 blood samples were delected by ELISA,there were 112 TP and HBsAg reactive samples,accounting for 47.26%of the 237 unqualified samples of all blood borne diseases.There were 60 HBsAg reactive specimens and 52 TP reactive specimens.Reactive samples were detected by colloidal gold method.HBsAg was positive in 27 cases(45%)and negative in 33 cases(55%);TP was positive in 37 cases(71.15%)and negative in 15 cases(28.85%).There was significant diference in the proportion of positive results detected by gold standard method with diferent S/CO values(P<0.05),There was significant difference in the proportion of TP and HBsAg positive in diferent blood collection numbers in external collection and blood room(P<0.05).The proporion of missing TP and HBsAg was 32.81%when there were more than 16 people in the blood room,and 45.31%when there were more than 100 people in the blood room.Conc lusion There is a certain proportion of missing detection in the delection of TP and HBsAg by colloidal gold method.The missing detection rate of TP and HBsAg can be further reduced by contrlling the key points in the primary screening link,strengthening the vocational skll training of staff and ensuring the staffing of blood donation sites.
