摘要
目的探讨精细化接种流程提高社区老年2型糖尿病患者新冠疫苗接种行为的效果。方法2021年3月30日至4月30日,采用方便抽样法,使用新冠疫苗犹豫行为调查问卷对在上海市浦东新区泥城社区卫生服务中心“糖友之屋”建档的128名社区老年2型糖尿病患者进行调查,分析影响新冠疫苗犹豫的相关因素,并通过调查结果制定出精细化接种流程,包括接种前评估、接种时观察、接种后随访的“三阶段九步骤”法,对所有调查对象进行干预,比较干预前后新冠疫苗犹豫率变化,干预后新冠疫苗接种情况及对接种流程的满意度情况。结果128名调查对象平均年龄(66.35±5.83)岁,男女比例为1.13∶1,发生疫苗犹豫的调查对象占25.00%;疫苗犹豫的原因中,认为身体有基础疾病、担心疫苗有副作用、认为家人疫苗犹豫的态度对我影响很大占比最高,分别为47.66%、45.31%、42.97%,且疫苗犹豫与性别、文化程度有关(P<0.05);实施精细化接种流程后,疫苗犹豫率从25.00%降低到6.25%,差异有统计学意义(P<0.05);有118例患者完成了第一针新冠疫苗的注射,且对整个接种流程的满意度达96%。结论精细化接种流程能有效降低新冠疫苗犹豫率,提升新冠疫苗接种率及患者满意度。
Objective To explore the effect of refined vaccination process in improving the COVID-19 vaccination behavior of elderly patients with type 2 diabetes in the community.Methods From March 30 to April 30,2021,128 elderly patients with type 2 diabetes recorded in“Diabetes Friends House”in Shanghai Pudong New Area Nicheng Community Health Service Center were investigated by convenience sampling method and the hesitation behavior questionnaire of COVID-19 vaccination.The related factors affecting the hesitation of COVID-19 vaccination were analyzed,and the refined vaccination process was developed through the survey results,including evaluation before vaccination,observation during vaccination,and the“three-stage and nine-step method”for follow-up after inoculation.All the respondents were intervened,and the changes in the hesitation rate of vaccination before and after the intervention were compared,as well as the inoculation status after the intervention and the satisfaction with the vaccination process.Results A total of 128 respondents were surveyed,with an average age of 66.35±5.83 and a male-to-female ratio of1.13:1,and 25.00%of the respondents had hesitation for COVID-19 vaccination.Underlying diseases,concern of side effects of vaccination,and influence by the family member’s hesitation were the main reasons for hesitation,accounting for 47.66%,45.31%,and 42.97%,respectively.Moreover,hesitation of vaccination was related to gender and educational level(P<0.05).After the implementation of the refined vaccination process,the hesitation rate decreased from 25.00%to 6.25%,and the difference was statistically significant(P<0.05).A total of 118 patients have completed the first dose of COVID-19 vaccination,and their satisfaction rate with the entire vaccination process was 96%.Conclusion Refined vaccination process can effectively reduce the hesitation rate of COVID-19 vaccination,and improve the vaccination rate and patients’satisfaction with vaccination process.
