摘要
术后睡眠障碍(postoperative sleep disturbance,PSD)是术后脑功能障碍的表现之一,可对术后康复产生不利影响,如谵妄风险、疼痛敏感性及心血管事件发生率增加等,逐渐成为值得关注的围手术期并发症之一.星状神经节阻滞(stel-late ganglion block,SGB)可通过抑制交感神经节兴奋性、调节自主神经功能而发挥改善睡眠的作用.文章总结了既往SGB对PSD的治疗方案及效果,从睡眠障碍的发生原因来探讨SGB改善PSD的作用机制,尤其是围绕褪黑素分泌、应激反应、脑血管舒缩及炎症反应等方面进行阐述,以求为PSD的预防及治疗提供有力依据.
Postoperative sleep disturbance(PSD)is one of the manifestations of postoperative brain dysfunction,which can have negative impacts on postoperative recoveries,such as the increased risk of delirium,pain sensitivity,and cardiovascular events,and is becoming a concern as a perioperative complication.Stellate ganglion block(SGB)has been shown to improve sleep quality by inhibiting sympathetic ganglion excitability and modulating autonomic function.This review summarized the previous treatment protocols and the fets of SCB on PSD,and discussed the mechanisms of SGB in improving PSD from the perspective of the causes of sleep disorders,especially in the areas of melatonin secretion,stress response,cerebral vasomotor and inflammatory response,in order to provide a strong basis forthe prevention and treatment of PSD.
