摘要
目的探究腹壁脂肪层自然对合在肥胖产妇剖宫产手术中的应用效果。方法选取2019年3月至2021年3月于本院行剖宫产手术的60名肥胖产妇作为研究对象,按照随机数字表法分为对照组和观察组,每组30名。对照组采用剖宫产后间断缝合处理,观察组采用剖宫产后腹壁脂肪层自然对合处理,比较两组术后不同时间视觉模拟评分法(VAS)评分、手术切口脂肪液化发生率、切口甲级愈合率及生命质量。结果术后第2、3、5天,观察组VAS评分均低于对照组,差异有统计学意义(P<0.05)。观察组手术切口出现脂肪液化发生率(6.67%)明显低于对照组(26.67%),切口甲级愈合率(93.33%)明显高于对照组(70.00%),差异有统计学意义(P<0.05)。观察组体能、精力、角色功能、社会功能、心理健康、躯体功能、身体疼痛、总体健康评分均高于对照组,差异有统计学意义(P<0.05)。结论腹壁脂肪层自然对合应用于肥胖剖宫产手术患者效果较好,有助于预防术后发生切口脂肪液化,减轻患者术后疼痛程度,提高切口甲级愈合率及生命质量,值得临床推广应用。
Objective To explore the application effect of natural involution of abdominal fat layer in obese puerperae in cesarean section.Methods A total of 60 obese patients with cesarean section who were treated in our hospital from March 2019 to March 2021 in our hospital were selected as the study subject,and they were divided into the control group and the observation group according to the random number table method,with 30 cases in each group.The control group was treated with intermittent suture after cesarean section,while the observation group was treated with natural involution of the abdominal fat layer after cesarean section,the visual analogue scale(VAS)score at different time after operation,the in-cidence of fat liquefaction in the surgical incision,the incision grade A healing rate and the quality of life were compared between the two groups.Results On the 2nd,3rd,and 5th days,the VAS scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).The incidence of fat liquefaction in the surgical incision in the observation group(6.67%)was significantly lower than that in the control group(26.67%),and the incision grade A healing rate(93.33%)was significantly higher than that in the control group(70.00%),and the difference was statistically significant(P<0.05).The physical fitness,energy,role function,social function,mental health,physical function,physical pain,and overall health scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The natural involution of the abdominal wall fat layer is effective in obese patients with cesarean section,can help prevent postoperative incision fat liquefaction,reduce postoperative pain,and improve incision grade A healing rate and quality of life,it is worthy of clinical application.
