Investigation on the Current Situation and Development Status of Medical Record Coders in General Hospitals of Second Level or Above in Guangdong Province after DIP Implementation

摘要 目的 本文通过对DIP实施后广东省病案编码人员发展现状调查和分析,探索加强病案编码队伍建设的路径。方法 通过全国医疗质量数据抽样调查,获取2018年和2020年度广东省二级以上综合医院病案管理专业调查数据。纳入本次研究的医疗机构共196家,提取并分析病案编码人员数量、结构、工作负荷等发展现状及变化趋势。结果 2020年样本机构平均编码人员数3.96人,与2018年相比增长39.93%;2020年编码员人年均负担病历数9700份/人年,与2018年相比下降18.5%。与2018年相比,2020年医学相关专业人员占比提高6.61%,达69.33%;本科及以上学历人员占比提高9.9%,达63.66%,但人员职称结构变化不明显。结论 编码人员数量增加,人均工作量下降,但各医院存在明显差异,应加强病案学科建设及专业人才培养、在职人员继续教育,完善病案职称晋升渠道,以缩小不同机构间病案队伍差异,加快建成一支专业素质高、业务能力强的编码队伍。 Objectives The investigation aims to study the current situation and development status of the medical record coders in Guangdong Province after DIP implementation in order to explore the approaches to strengthen the construction of the medical record coding team. Methods A network sampling survey on national medical care quality was conducted through National Clinical Improvement System. The survey data were obtained from general hospitals of secondary level and above in Guangdong province in 2018 and 2020. A total of 196 medical institutions were selected. Medical record coding personnel’s development status and trend, such as quantity,composition and workload, were analyzed. Results The average number of coders in the sampled institutions in 2020was 3.96, which was up by 39.93% compared to 2018.In 2020, the average number of medical records borne by each coder was 9,700 per year, which was down by 18.5% compared with 2018. In comparison with 2018, the proportion of medical-related professionals in 2020 increased by 6.61% and reached 69.33%, and the proportion of those with bachelor’s degree or above increased by 9.9% and reached 63.66%. However, the change in the composition of professional titles was not obvious. Conclusion Confronting with new opportunities and challenges,hospitals should strengthen medical record discipline construction, professional personnel training, continuing education of in-service personnel. They should also improve the promotion channels for professional title of medical record to reduce the differences among medical record teams from various institutions and accelerate the construction of a well-educated coding team with high professional qualifications.
