经食管超声心动图评估非瓣膜性心房颤动患者射频导管消融术前后左心耳形态与功能的价值 被引量:1

Application of trans-esophgeal echocardiography in evaluation of left atrial appendage morphological and functional changes before and after radiofrequency ablation in patients with non-valvular atrial fibrillation

摘要 目的探讨经食管超声心动图(TEE)评估非瓣膜性心房颤动(下称房颤)患者左心耳结构及功能的价值。方法选择2019年1月至2021年3月在余姚市人民医院接受射频导管消融术的房颤患者213例,分别于治疗前后采用TEE测量左心耳长径、左心耳口长径和短径、左心耳口面积,左心耳射血分数(LAAEF)、峰值血流排空速率(PEV)、舒张末期容积(EDV)、收缩末期容积(ESV)及二尖瓣对合面积和对合指数。结果与治疗前比较,治疗后的左心耳口短径缩短、LAA EF、PEV增大,EDV缩小,差异均有统计学意义(均P<0.05)。治疗后对合指数较治疗前改善,差异有统计学意义(P<0.05)。治疗后对合面积改善不明显。结论TEE可用于评估房颤射频导管消融术后左心耳的结构和功能变化。 Objective To investigate value of trans-esophageal echocardiography(TEE)in evaluating the structure and function of left atrial appendage(LAA)in patients with non-valvular atrial fibrillation(AF).Methods A total of 213 AF patients who underwent radiofrequency catheter ablation in Yuyao People's Hospital from January 2019 to March 2021 were selected.TEE was performed to measure LAA length,the maximum and minimum LAA ostial dimension,LAA ostial area,left atrial appendage ejection fraction(LAAEF),peak emptying velocity(PEV),end-diastolic volume(EDV),endsystolic volume(ESV),mitral valve coaptation zone area and coaptation index before and after treatment.Results The minimum LAA ostial dimension was significantly smaller,LAA EF and PEV were significantly higher and EDV was significantly lower after treatment than those before treatment(all P<0.05).Coaptation index was improved significantly after treatment than before(P<0.05).Coaptation area was not improved after treatment.Conclusion TEE can be used to evaluate the changes of LAA structure and function after radiofrequency ablation of AF.
