摘要
目的探讨血液常规分析衍生炎性参数指标[中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、系统性免疫性炎症指数(SII)、中性粒细胞、单核细胞乘积与淋巴细胞比值(NMLR)、红细胞分布宽度(RDW)]在抗中性粒细胞胞浆抗体相关性血管炎(anti-neutrophil cytoplasmic antibody associated vasculitis,AAV)中的临床意义。方法选择2017年1月至2021年10月我院初诊的52例AAV患者为研究对象,50例健康体检人群为对照组,分析AAV患者血液常规分析衍生炎性参数NLR、PLR、SII、NMLR、RDW及其他血液炎性指标的变化及相关性。结果AAV患者SII、NLR、PLR、NMLR以及RDW均显著高于健康对照组(P<0.01),PDW结果与健康对照组比较差异无统计学意义(P>0.05);Spearman相关性分析显示,AAV患者NMLR与PA(r=-0.3589,P=0.009)呈负相关,PLR与ESR(r=0.3910,P=0.0042)和α1球蛋白(r=0.3715,P=0.0067)呈正相关,与PA(r=-0.3231,P=0.0195)呈负相关;SII水平与α1球蛋白(r=0.4520,P=0.0008)、CRP(r=0.5014,P=0.0002)、ESR(r=0.3188,P=0.0212)呈正相关,与PA(r=-0.4801,P=0.0003)呈负相关;RDW水平与α1球蛋白(r=0.3306,P=0.0167)、NLR(r=0.3238,P=0.0192)、NMLR(r=0.3945,P=0.0038)呈正相关。结论以循环血液中细胞数量、细胞体积异质性为核心衍生的NLR、SII、NMLR、PLR、RDW等参数是一种简便、易行的炎症参数,在反映AAV疾病活动方面具有潜在的临床应用前景,有望为AAV患者的临床管理提供有价值的帮助。
Objective To study the value of neutrophil lymphocyte ratio(NLR),monocyte to lymphocyte ratio(MLR),platelet to lymphocyte ratio(PLR),systemic immune inflammatory index(SII),neutrophil monocyte lymphocyte ratio(NMLR),red blood cell distribution width(RDW)derived from routine blood examination and to evaluate the state of inflammation in anti-neutrophil cytoplasmic antibody associated vasculitis.Methods 52 patients were recruited and retrospective observationally analyzed in our department from January,2017 to June,2021,while 50 healthy subjects were enrolled to the study as control.The changes of inflammatory parameters NLR,PLR,SII,NMLR,and RDW derived from routine blood analysis and the correlation with other inflammatory indicators in healthy subjects and patients with AAV were analyzed.Results SII,NLR,PLR,NMLR and RDW in AAV patients were significantly higher than those in the healthy control group(P<0.01),while PDW results showed no significant difference between AAV patients and the healthy control group(P>0.05).Spearman correlation analysis showed that NMLR was negatively correlated with PA(r=-0.3589,P=0.009),while PLR was positively correlated with ESR(r=0.3910,P=0.0042)andα1 globulin(r=0.3715,P=0.0067),negatively correlated with PA(r=-0.3231,P=0.0195).SII level was positively correlated withα1 globulin(r=0.4520,P=0.0008),CRP(r=0.5014,P=0.0002),ESR(r=0.3188,P=0.0212),and negatively correlated with PA(r=-0.4801,P=0.0003).RDW level was positively correlated withα1 globulin(r=0.3306,P=0.0167),NLR(r=0.3238,P=0.0192)and NMLR(r=0.3945,P=0.0038).Conclusion NLR,SII,NMLR,PLR,and RDW derived from cell number or cell volume heterogeneity in circulating blood are routinely collected and feasible inflammatory parameters,which have potential clinical application values in reflecting disease activity of AAV and are expected to provide a valuable help for the clinical management of patients with AAV.
