摘要
妊娠期糖尿病是妊娠期常见并发症,可引发早产、巨大儿、胎膜早破、新生儿低血糖、羊水过多及剖宫产等诸多不良妊娠结局。我国妊娠期糖尿病发病率不断上升,已经严重威胁到母婴健康。研究发现,维生素D可维持葡萄糖耐量的正常范围并提高胰岛素敏感性。妊娠期间孕妇在体内激素水平及胎儿骨骼生长发育等因素作用下,对于维生素D需求增高4~5倍,尤其在孕早期维生素D缺乏发生率较高。孕妇妊娠期间补充维生素D可有效降低妊娠期糖尿病发生风险、减少母婴不良结局的发生,为妊娠安全提供保障。本文对近年来维生素D与妊娠期糖尿病妊娠结局相关研究进行综述,包括不同剂量维生素D缺乏、不同补充途径、不同补充时间以及联合不同辅助治疗对于妊娠期糖尿病发病率及母婴结局影响的研究,旨在为维生素D在临床应用中改善妊娠期糖尿病不良母婴结局提供科学依据。
Gestational diabetes mellitus is a common complication of pregnancy,which can cause many adverse pregnancy outcomes,such as premature delivery,macrosomia,premature rupture of membranes,neonatal hypoglycemia,polyhydramnios and cesarean section.The incidence of gestational diabetes mellitus in China is increasing,which has seriously threatened maternal and infant health.Vitamin D has been found to maintain a normal range of glucose tolerance and improve insulin sensitivity.During pregnancy,pregnant women′s demand for vitamin D increases 4-5 times under the action of hormone levels and fetal bone growth and development,especially in the first trimester,the incidence of vitamin D deficiency is higher.Vitamin D supplementation during pregnancy can effectively reduce the risk of gestational diabetes,reduce the incidence of maternal and infant adverse outcomes,and provide a guarantee for the safety of pregnancy.This article reviews the recent studies on the relationship between vitamin D and pregnancy outcomes of gestational diabetes mellitus,included the effect of different doses of vitamin D deficiency,different ways of supplementation,different time of supplementation,and combined with different adjuvant therapy on the incidence of gestational diabetes mellitus and maternal and infant outcomes,to provide a scientific basis for the clinical application of vitamin D in improving adverse maternal and infant outcomes of gestational diabetes mellitus.
