Prenatal ultrasonic manifestations of caudal regression syndrome

摘要 目的观察胎儿尾部退化综合征(CRS)产前超声表现。方法回顾性分析32胎CRS产前超声资料,观察其超声表现及伴发异常。结果32胎中,7胎于妊娠早期(<孕14周)、25胎于妊娠中晚期(14~40周)检出;均表现为脊柱尾部发育不良伴不同程度椎体缺如,其中27胎(27/32,84.38%)骶尾椎受累,31胎(31/32,96.88%)合并其他系统异常。7胎于妊娠早期检出的CRS胎儿均合并其他系统异常,其中5胎肢体异常、4胎颈部透明层增厚。25胎于妊娠中晚期检出的CRS中,14胎(14/25,56.00%)合并肛门闭锁,12胎(12/25,48.00%)合并肾脏异常,8胎(8/25,32.00%)肢体受累;其中13胎可见脊髓末端,8胎脊髓拴系。随访中30例妊娠终止,1胎失访,1胎活产。结论CRS产前超声表现具有特异性,且多合并其他系统畸形。 Objective To observe the prenatal ultrasonic features of caudal regression syndrome(CRS).Methods Data of prenatal ultrasound of 32 fetuses with CRS were retrospectively analyzed,the ultrasonic manifestations and associated malformations were observed.Results Among 32 fetuses with CRS,7 were detected in the first trimester(<14 weeks),while 25 were diagnosed in the second and third trimester(14—40 weeks).Caudal dysplasia and varying degrees of vertebral absence were observed in all 32 fetuses,the involved sacral coccygeal vertebras were noticed in 27(27/32,84.38%)fetuses,while other complicated malformations were found in 31 fetuses(31/32,96.88%).All 7 CRS fetuses diagnosed in the first trimester were associated with other systemic abnormalities,including limb abnormalities in 5 fetuses and nuchal translucency thickening in 4 fetuses.Among 25 fetuses detected in the second and the third trimester,complicated digestive system abnormalities(all anal atresia)were observed in 14 fetuses(14/25,56.00%),urinary system abnormalities(all renal abnormalities)in 12 fetuses(12/25,48.00%),while limb abnormalities were found in 8 fetuses(8/25,32.00%).Among the above 25 fetuses,the end of the spinal cord could be depicted in 13,which became tethered in 8 fetuses.During following-up,elective termination of pregnancy was chosen in 30 cases,1 fetus was lost and 1 was born alive.Conclusion CRS had typical prenatal ultrasonic characteristics,often associated with other systemic malformations.
