三维超声成像技术比较阴道分娩与剖宫产分娩对盆底结构的影响 被引量:3

摘要 目的:运用三维超声成像技术比较阴道分娩与剖宫产分娩对盆底结构的影响。方法:回顾性分析2020年1—12月于该院分娩的116例产妇的临床资料,根据分娩方式分为研究组67例和对照组49例。研究组经阴道分娩,对照组经剖宫产分娩,于产后6周门诊复查,分别于静息、Valsalva状态下完成经会阴三维超声检查。比较两组静息、Valsalva状态下盆底结构指标(肛提肌裂孔面积、前后径、左右径)水平,静息状态下膀胱颈位置、Valsalva状态下盆腔三维超声成像指标[膀胱颈移动度(BND)、尿道内口漏斗形成率、膀胱膨出率]水平。结果:产后6周,研究组静息、Valsalva状态下肛提肌裂孔面积、前后径、左右径均大于对照组,差异有统计学意义(P<0.05);两组静息状态下膀胱颈位置比较,差异无统计学意义(P>0.05);研究组Valsalva状态下尿道内口漏斗形成率、膀胱膨出率均高于对照组,BND大于对照组,差异有统计学意义(P<0.05)。结论:经三维超声成像技术评估,阴道分娩对产妇盆底结构的影响大于剖宫产分娩。 Objective:To compare effects of vaginal and cesarean section delivery on pelvic floor structure by three-dimensional ultrasound imaging.Methods:The clinical data of 116 puerperae who gave birth in this hospital from January to December 2020 were retrospectively analyzed.According to the mode of delivery,they were divided into study group(67 cases)and control group(49 cases).The study group delivered vaginally,while the control group delivered by cesarean section.The outpatient review occurred 6 weeks after delivery.Transperineal three-dimensional ultrasound imaging was performed at resting and Valsalva states,respectively.The levels of pelvic floor structure indicators(levator hiatus area,anterior-posterior diameter,left-right diameter),the position of the bladder neck at resting state,and the three-dimensional ultrasound imaging index of the pelvic cavity at the maximum Valsalva state[bladder neck descend(BND),internal urethral orifice funnel formation rate,cystocele rate]were compared between the two groups.Results:6 weeks after delivery,the levator hiatus area,the anterior-posterior diameter,and the left-right diameter of the study group at rest and at Valsalva state were greater than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the position of the bladder neck between the two groups at rest(P>0.05).Further,the internal urethral orifice funnel formation rate and the cystocele rate in the study group were higher than those in the control group at maximal Valsalva state;the BND was greater than that in the control group;and the differences were statistically significant(P<0.05).Conclusions:As assessed by three-dimensional ultrasound imaging,vaginal delivery has a greater effect on puerperae pelvic floor structures than cesarean section delivery.
