摘要
目的 观察辛伐他汀辅助治疗对子宫内膜异位症(EMS)术后患者氧化应激状态及胰岛素样生长因子-1(IGF-1)、血管内皮生长因子(VEGF)水平的影响。方法 选取2018年12月—2020年12月延安市人民医院妇产科诊治的EMS患者108例作为研究对象,以简单随机化法分为对照组、研究组,各54例。对照组采取腹腔镜手术联合醋酸戈舍瑞林缓释植入剂治疗,研究组在对照组基础上加用辛伐他汀,均治疗6个月。比较2组疗效,治疗前后Kupperman评分、主观感受评分(阳性体征评分、盆腔症状评分、总主观感受评分)、血清氧化应激指标[活性氧簇(ROS)、维生素E(VE)、超氧化物歧化酶(SOD)]、IGF-1、VEGF水平,以及不良反应发生率、复发率。结果 研究组完全缓解率高于对照组(94.44%vs.79.63%,χ~2/P=5.252/0.022);治疗6个月后,研究组Kupperman评分、阳性体征评分、盆腔症状评分、总主观感受评分低于对照组(t=5.683、11.073、10.155、11.797,P均<0.001);研究组血清ROS、IGF-1、VEGF水平低于对照组,血清VE、SOD水平高于对照组(t=4.009、7.010、4.236、5.168、5.286,P均<0.001);2组不良反应发生率及复发率比较,差异无统计学意义(P>0.05)。结论 辛伐他汀辅助治疗EMS患者,可降低血清IGF-1、VEGF水平,改善氧化应激状态,疗效显著,且安全性高。
Objective To observe the effects of simvastatin on oxidative stress,insulin like growth factor-1(IGF-1) and vascular endothelial growth factor(VEGF) levels in patients with endometriosis(EMS) after operation.Methods One hundred and eight EMS patients diagnosed and treated in the Obstetrics and Gynecology Department of Yan’an People’s Hospital from December 2018 to December 2020 were selected as the research objects,and were divided into the control group and the research group by simple randomization,with 54 patients in each group.The control group was treated with laparoscopy combined with Goserellin acetate sustained-release implant,while the study group was treated with simvastatin on the basis of the control group for 6 months.The curative effects of the two groups were compared,including Kupperman score,subjective feeling score(positive sign score,pelvic symptom score,total subjective feeling score),serum oxidative stress indicators [ROS,vitamin E,superoxide dismutase(SOD)],IGF-1,VEGF levels before and after treatment,as well as the incidence and recurrence rate of adverse reactions.Results The complete remission rate in the study group was higher than that in the control group(94.44% vs.79.63%,χ~2/P=5.252/0.022).After 6 months of treatment,Kupperman score,positive sign score,pelvic symptom score and total subjective feeling score in the study group were lower than those in the control group(t=5.683,11.073,10.155,11.797,P0.05).Conclusion Simvastatin can reduce serum IGF-1 and VEGF levels and improve oxidative stress status in patients with EMS,with significant efficacy and high safety.
