摘要
目的:分析核磁共振用于诊断膝关节外伤性骨软骨骨折的诊断有效性及价值,为临床疾病的确诊及治疗方案的制定提供可靠的参考依据。方法:将2016年1月~2021年12月本院收治的112例疑似膝关节外伤性骨软骨骨折患者作为研究诊断对象,针对112例疑似膝关节外伤性骨软骨骨折患者均实施核磁共振的诊断检查,以最终的手术病理学诊断结果作为金标准(112例疑似患者经手术病理学诊断确诊的患者为100例),分析核磁共振用于诊断膝关节外伤性骨软骨骨折的诊断价值。结果:核磁共振诊断膝关节外伤性骨软骨骨折的敏感度为99.00%,特异度为83.33%,准确性为97.32%,与手术病理学诊断结果相比统计学无意义(P>0.05);其中髌骨软骨骨折患者19例,股骨外踝骨软骨骨折患者81例,其中合并关节外侧半月板损伤患者15例,合并关节前后交叉韧带损伤患者12例。结论:在对膝关节外伤性骨软骨骨折患者实施诊断的过程中给予核磁共振诊断检查的应用价值较高,能最大程度及清晰地将骨软骨骨折的严重程度、骨折具体部位、是否存在游离骨块及合并损伤情况反映出来,为临床手术方案的制定和实施提供可靠的参考依据,促进患者预后。
Objective:To analyze the effectiveness and value of MRI in the diagnosis of traumatic osteochondral fractures of the knee,so as to provide a reliable reference for the diagnosis of clinical diseases and the formulation of treatment plans.Methods:112 patients with suspected knee traumatic osteochondral fracture treated in our hospital from January 2016 to December 2021 were studied and diagnosed.112 patients with suspected knee traumatic osteochondral fracture were examined by MRI,the diagnostic value of MRI in the diagnosis of traumatic osteochondral fracture of knee joint was analyzed with the final surgical pathological diagnosis results as the gold standard(112 suspected patients and 100 patients diagnosed by surgical pathology).Results:The sensitivity,specificity and accuracy of MRI in the diagnosis of traumatic osteochondral fracture of knee joint were 99.00%,83.33%and 97.32%,which were statistically insignificant compared with the results of surgical pathology(P>0.05);There were 19 patients with patellar cartilage fracture,81 patients with femoral lateral ankle osteocartilage fracture,including 15 patients with lateral meniscus injury and 12 patients with anterior and posterior cruciate ligament injury.Conclusion:MRI is of high value in the diagnosis of patients with traumatic osteochondral fracture of knee joint.It can reflect the severity of osteochondral fracture,specific position of fracture,free bone mass and combined injury to the greatest extent and clearly,and provide a reliable reference for the formulation and implementation of clinical operation plan,promote patient prognosis.
