摘要
目的探讨血液灌流(HP)联合血液透析(HD)治疗急诊重症有机磷农药中毒的临床效果。方法选取2019年2月至2021年2月我院收治的106例急诊重症有机磷农药中毒患者为研究对象,采用电脑盲选法将其分为对照组和试验组,每组53例。对照组给予常规治疗,试验组在此基础上给予HP+HD治疗。比较两组的临床治疗效果。结果治疗3 d后,试验组的肌钙蛋白I(cTnI)、脑钠肽(BNP)、单核细胞趋化蛋白-1(MCP-1)、白细胞介素-6(IL-6)、转化生长因子-β1(TGF-β1)水平显著低于对照组,左心室射血分数(LVEF)高于对照组(P<0.05)。试验组的清醒时间、胆碱酯酶活性恢复正常时间、住院天数短于对照组,阿托品使用总量少于对照组,并发症发生率低于对照组(P<0.05)。治疗3 d后,试验组的血红蛋白、前白蛋白、血清白蛋白水平显著高于对照组(P<0.05)。结论HP+HD治疗急诊重症有机磷农药中毒患者的效果显著,可减轻心肌损伤,改善营养状况,促进疾病康复。
Objective To investigate the clinical effect of hemoperfusion(HP)combined with hemodialysis(HD)in the treatment of severe acute organic phosphorus pesticide poisoning.Methods A total of 106 patients with severe acute organic phosphorus pesticide poisoning admitted in our hospital from February 2019 to February 2021 were selected as the research objects.The patients were divided into control group and experimental group by computerized blind selection method,with 53 cases in each group.The control group was given conventional treatment,and the experimental group was given HP+HD treatment on this basis.The clinical treatment effect of the two groups was compared.Results After 3 d of treatment,the levels of cardiac troponin I(cTnI),brain natriuretic peptide(BNP),monocyte chemotactic protein-1(MCP-1),interleukin-6(IL-6)and transforming growth factor-β1(TGF-β1)of the experimental group were significantly lower than those of the control group,and the left ventricular ejection fraction(LVEF)was higher than that of the control group(P<0.05).The awake time,the recovery time of cholinesterase activity and the hospital stay in the experimental group were shorter than those in the control group,the total amount of atropine used was less than that in the control group,and the incidence of complications was lower than that in the control group(P<0.05).After 3 d of treatment,the levels of hemoglobin,prealbumin and serum albumin in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion HP+HD in the treatment of patients with severe acute organic phosphorus pesticide poisoning has a significant effect,which can reduce myocardial injury,improve nutritional status,and promote disease rehabilitation.
