摘要
目的分析不同类型菌血症患者血清降钙素原(PCT)、C-反应蛋白(CRP)及白细胞介素-6(IL-6)的检出结果及意义。方法选择我院50例菌血症患者。统计记录PCT、CRP、IL-6分别在菌血症患者中的阳性检出率;分析革兰氏阳性菌、革兰氏阴性菌感染患者的PCT、CRP、IL-6检出结果;比较一般耐药、多重耐药及广泛耐药患者的PCT、CRP、IL-6检出结果。结果IL-6在菌血症患者中的阳性检出率高于PCT和CRP的阳性检出率(P<0.05)。50例菌血症患者共检出革兰氏阳性菌感染31例,革兰氏阴性菌感染19例。革兰氏阴性菌感染患者的PCT、CRP、IL-6水平显著高于革兰氏阳性菌感染患者(P<0.05)。革兰氏阳性菌感染患者的IL-6阳性检出率高于CRP阳性检出率(P<0.05)。50例菌血症患者中,检出一般耐药11例,多重耐药25例,广泛耐药14例。三种不同耐药程度患者的IL-6水平存在显著差异(P<0.05)。广泛耐药患者的IL-6水平高于多重耐药、一般耐药患者(P<0.05)。一般耐药患者的IL-6阳性检出率高于PCT及CRP阳性检出率(P<0.05)。结论血清PCT、CRP、IL-6在菌血症患者中的阳性检出率均较高,在不同菌种感染判断中,三者均有一定的价值,而在不同耐药程度判断方面IL-6的价值更高,因此在临床血液细菌培养前,可考虑综合三者进行菌血症的感染菌种、耐药程度判断。
Objective To analyze the detection results and significance of serum procalcitonin(PCT),C-reactive protein(CRP)and interleukin-6(IL-6)in patients with different types of bacteremia.Methods A total of 50 patients with bacteremia admitted in our hospital were selected.The positive detection rates of PCT,CRP and IL-6 in patients with bacteremia were statistically recorded;the detection results of PCT,CRP and IL-6 in patients with Gram-positive bacterial infections and Gram-negative bacterial infections were analyzed;the detection results of PCT,CRP and IL-6 in patients with general drug resistance,multiple drug resistance and extensive drug resistance were compared.Results The positive detection rate of IL-6 in patients with bacteremia was higher than the positive detection rates of PCT and CRP(P<0.05).A total of 31 cases of Grampositive bacteria and 19 cases of Gram-negative bacteria were detected in 50 patients with bacteremia.The levels of PCT,CRP and IL-6 in patients with Gram-negative bacteria infections were significantly higher than those with Gram-positive bacteria infections(P<0.05).The positive detection rate of IL-6 in patients with Gram-positive bacterial infections was higher than the positive detection rate of CRP(P<0.05).Among 50 patients with bacteremia,11 cases of general drug resistance,25 cases of multidrug resistance and 14 cases of extensive drug resistance were detected.There was significant difference in IL-6level among patients with three different degrees of drug resistance(P<0.05).The level of IL-6 in patients with extensive drug resistance was higher than that in patients with multiple drug resistance and general drug resistance(P<0.05).The positive detection rate of IL-6 in patients with general drug resistance was higher than the postive detection rates of PCT and CRP(P<0.05).Conclusion The positive detection rates of serum PCT,CRP and IL-6 in patients with bacteremia were higher.In the judgment of different strains infections,the three has a certain value,and the value of IL-6 is higher in the judgment of different degrees of drug resistance.Therefore,the three can be considered to judge the infection strains and drug resistance of bacteremia before clinical blood bacterial culture.
