摘要
目的探讨清热渗湿汤结合痔瘘洗剂熏洗坐浴对湿热下注型肛窦炎(AS)患者中医症候积分、血清8-羟基脱氧鸟苷(8-OHDG)水平及复发率的影响。方法将通用环球中铁西安医院2018年6月至2021年5月收治的86例湿热下注型AS患者按随机数表法分为观察组和对照组,每组43例。所有患者均服用左氧氟沙星片,对照组在此基础上应用清热渗湿汤治疗,观察组在对照组治疗的基础上加用痔瘘洗剂熏洗坐浴治疗,两组患者均连续治疗2周。比较两组患者的疗效,治疗前后的中医证候积分、血清炎症指标[肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)]、氧化应激指标[丙二醛(MDA)、8-羟基脱氧鸟苷(8-OHDG)]和停药后的复发率。结果观察组患者的治疗总有效率为95.35%,明显高于对照组79.07%,差异有统计学意义(P<0.05);治疗后,两组患者的中医证候积分、血清TNF-α、PCT、MDA、8-OHDG水平和治疗前相比均明显降低,且观察组中医证候积分、血清TNF-α、PCT、MDA、8-OHDG水平均明显低于对照组,差异有统计学意义(P<0.05);观察组停药2个月、4个月均未见复发,对照组停药2个月、4个月复发率分别为6.98%、13.95%,观察组停药4个月复发率明显低于对照组,差异有统计学意义(P<0.05)。结论对湿热下注型AS患者实施清热渗湿汤结合痔瘘洗剂熏洗坐浴治疗的效果较佳,其可明显改善临床症状,有效减轻炎症、氧化应激反应,显著降低复发率。
Objective To investigate the effects of heat-clearing and dampness-draining decoction combined with hemorrhoid fistula lotion on TCM symptom score,serum 8-hydroxy-deoxyguanosine(8-OHDG)level,and recurrence rate in patients with anal sinusitis(AS)due to damp-heat pouring downward.Methods A total of 86 patients with AS due to damp-heat pouring downward who were admitted to General Global China Railway Xi'an Hospital from June 2018 to May 2021 were divided into control group(n=43)and treatment group(n=43)according to random number table method.Based on levofloxacin tablets,the patients in the control group were treated with heat-clearing and damp-ness-draining decoction,and the patients in the treatment group were treated with heat-clearing and dampness-draining decoction combined with hemorrhoid fistula lotion.Both groups were treated continuously for 2 weeks.The efficacy,TCM syndrome score,serum inflammation index[tumor necrosis factor-α(TNF-α),procalcitonin(PCT)],oxidative stress index[malondialdehyde(MDA),8-hydroxydeoxyguanosine(8-OHDG)],and recurrence rate after drug withdraw-al were compared between the two groups.Results The total effective rate in the treatment group was 95.35%,which was significantly higher than 79.07%in the control group(P<0.05).After treatment,the scores of TCM syndromes,se-rum TNF-α,PCT,MDA,8-OHDG levels in the two groups were significantly lower than those before treatment,and the levels in the treatment group were significantly lower than those in the control group,with statistically significant difference(P<0.05).There was no recurrence in the treatment group within 2 months and 4 months after drug with-drawal,and the recurrence rates of the control group were 6.98%and 13.95%,respectively;the recurrence rate of the treatment group was significantly lower than that in the control group within 4 months after drug withdrawal(P<0.05).Conclusion For patients with AS due to damp-heat pouring downward,the effect of heat-clearing and dampness-drain-ing decoction combined with hemorrhoid fistula lotion is better,which can significantly improve clinical symptoms,ef-fectively reduce inflammation and oxidative stress response,and significantly reduce the recurrence rate.
