摘要
目的分析鼻内镜下经筛窦眶纸板入路眶内肿瘤切除术的应用价值。方法回顾性病例对照研究。收集我院2018年6月至2020年6月收治的行切除手术治疗的眶内良性肿瘤肿瘤患者资料,共60例(60只眼)。其中30例采取传统前路开眶肿瘤切除手术的患者,纳入对照组;另外30例采取鼻内镜下经筛窦眶纸板入路肿瘤切除手术的患者纳入研究组。对比两组患者手术前后的视力、眼球突出度、眼轴长度变化,手术后随访6个月期间的并发症总发生率与复发率。结果两组患者在手术前的视力、眼球突出度、眼轴长度进行对比无统计学差异(P>0.05),研究组在术后的视力水平均升高,眼球突出度均缩短,眼轴长度均增加,且研究组在术后的眼球突出度短于对照组(P<0.05),眼轴长度长于对照组(P<0.05)。两组间的手术时间无统计学差异(P>0.05),研究组住院时间短于对照组(P<0.05)。研究组患者在手术后随访6个月期间的并发症总发生率低于对照组(P<0.05),而两组间的复发率无统计学差异(P>0.05)。结论在眶内肿瘤切除术中,采取鼻内镜下经筛窦眶纸板入路,可有效改善患者的视力状况,复发风险小,并发症少,手术效果显著且安全性高。
Objective To analyze the value of endoscopic resection of orbital tumor via ethmoid sinus orbital paperboard approach.Methods A retrospective case series study.60 patients with orbital benign tumor who were treated by resection in our hospital from June 2018 to June 2020 were included.Among them,30 patients who underwent traditional resection of anterior orbital tumor were included in the control group.Another 30 patients who underwent tumor resection through ethmoid sinus orbital paperboard approach under nasal endoscopy were included in the study group.The differences of visual acuity,exophthalmos and axial length were compared between the two groups before and after operation.All patients were followed up for 6 months,and the incidence and recurrence rate of complications were observed.Results There was no significant difference in preoperative visual acuity,exophthalmos and axial length between the two groups(P>0.05).The postoperative visual acuity of the study group increased,the exophthalmos shortened and the axial length increased,and the postoperative exophthalmos of the study group was shorter than that of the control group(P<0.05),and the axial length was longer than that of the control group(P<0.05).There was no significant difference in operation time between the two groups(P>0.05),and the hospital stay in the study group was shorter than that in the control group(P<0.05).The incidence of complications in the study group was lower than that in the control group(P<0.05),and there was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusions In the resection of orbital tumor the ethmoid sinus orbital paperboard approach under nasal endoscopy can effectively improve the visual acuity of patients,with low recurrence risk,less complications,significant surgical effect and high safety.
