摘要
目的探讨平均血小板体积(MPV)、纤维蛋白原/白蛋白比值(FAR)与冠状动脉病变程度的相关性。方法选取合肥市第二人民医院2018年1月至2021年12月住院并行冠状动脉造影术(CAG)的患者共511例。根据CAG结果分为冠心病组(334例)和对照组(177例),冠心病组根据病变血管支数分为单支、双支以及三支病变组;基于MPV、FAR中位数将所有患者分为高MPV和FAR组、低MPV和FAR组,比较两组间Gensini评分的差异。绘制MPV、FAR以及联合指标预测冠心病的ROC曲线,并确定最佳截点值。结果冠心病组的FAR水平[0.07(0.06,0.08)比0.06(0.05,0.07)]和MPV水平[10.9(10.2,11.7)比10.3(9.6,11.2)]均高于对照组,差异均具有统计学意义(均P<0.05);高MPV水平组的Gensini评分高于低MPV水平组[41(20,62)比27(12,47)],差异具有统计学意义(P<0.05),高FAR水平组Gensini评分高于低FAR水平组[42(26,62)比24(12,46)],差异具有统计学意义(P<0.05);Spearman相关性分析显示,FAR和MPV与Gensini评分正相关,相关系数分别为r_(FAR)=0.426,r_(MPV)=0.254(P<0.05);ROC曲线分析结果表明,FAR、MPV及两者联合指标均对冠心病具有预测价值,预测冠心病的曲线下面积分别为0.705(95%CI 0.664~0.745)、0.619(95%CI 0.576~0.662)、0.732(95%CI 0.691~0.770),二者联合预测冠心病的曲线下面积高于单个指标,差异具有统计学意义(P<0.05)。结论FAR、MPV与冠状动脉病变程度正相关,FAR、MPV可以预测冠状动脉病变程度,且二者联合对冠心病的预测价值大于单个指标。
Objective To investigate the correlation between mean platelet volume(MPV)and fibrinogen/albumin ratio(FAR)and the severity of coronary artery disease.Methods A total of 511 patients with Coronary angiography(CAG)were hospitalized in Hefei Second People’s Hospital from January 2018 to December 2021.According to the CAG results,the patients divided into the coronary heart disease group(n=334 cases)and the control group(n=177 cases);the coronary heart disease group was divided into the single,double and three lesion groups according to the number of the diseased vascular branches.Based on the median of MPV and FAR,the patients were divided into the high MPV and FAR group,low MPV and FAR group,and the differences between the 2 groups were compared.Plot the ROC curves of MPV,FAR and the combined indicators to predict coronary heart disease and determine the optimal cut-off point value.Results The FAR level of the coronary heart disease group was higher than that of the control group[0.07(0.06,0.08)vs.0.06(0.05,0.07)],and the difference was statistically significant(P<0.05);the MPV level of the coronary heart disease group was higher than that of the control group[10.9(10.2,11.7)vs.10.3(9.6,11.2)],and the difference was statistically significant(P<0.05).The Gensini score in the high MPV level group was higher than that in the low MPV level group[41(20,62)vs.27(12,47)],the difference was statistically significant(P<0.05),and the Gensini score in the high FAR level group was higher than that in the low FAR level group[42(26,62)vs.24(12,46)],and the difference was statistically significant(P<0.05);Spearman correlation analysis showed that MPV and FAR and Gensini scores were positively correlated,and the correlation coefficients were r_(FAR)=0.426,r_(MPV)=0.254,(P<0.05).The results of ROC curve analysis showed that FAR and MPV and the combined indicators of the two had predictive value for coronary heart disease;and the area under the curve predicted coronary heart disease were 0.705(95%CI 0.664~0.745),0.619(95%CI 0.576~0.662)and 0.732(95%CI 0.691~0.770),respectively,The area under the curve of the two combined predictions of coronary heart disease was higher than that of a single index,and the difference was statistically significant(P<0.05).Conclusions There is a correlation between FAR and MPV and the severity of coronary artery disease;FAR and MPV can predict the degree of coronary artery disease,and the predictive value of the combination of the two is greater than any a single index.
