摘要
目的探究集束化护理在老年股骨颈骨折手术患者中的应用效果。方法选取2019年9月至2022年9月福建省浦城县医院ICU诊治的150例老年股骨颈骨折手术患者,以简单随机抽样方法将其随机分为两组,即试验组(奇数组,75例)和对照组(偶数组,75例)。对照组采用常规护理,试验组在此基础上采用集束化护理。针对两组患者护理前后的抑郁自评量表(SDS)、焦虑情绪自评量表(SAS)、疼痛评估量表(VAS),以及护理满意度及并发症情况进行观察与比较。结果干预后试验组患者的SDS(29.20±1.40)分、SAS(28.90±4.70)分、显著低于对照组(40.40±4.30)分、(40.30±4.50)分,组间差异具有统计学意义(t=30.333,21.457,P<0.05)。术后24 h、72 h时,试验组VAS评分(3.20±0.70)分、(2.80±0.60)分显著低于对照组(6.60±0.80)分、(4.60±0.50)分,差异有统计学意义(t=39.173,28.226,P<0.05)。试验组患者满意度为96%,对照组满意度为84%,组间差异具有统计学意义(χ^(2)=6.000,P<0.05);试验组患者并发症发生率4.00%明显低于对照组25.33%(χ^(2)=13.636,P<0.05)。结论集束化护理能够有效改善老年股骨颈骨折手术患者的不良心理状态,缓解疼痛症状,降低并发症风险,提高护理满意度,值得临床上推广应用。
Objective To explore the application effect of cluster nursing in elderly patients with femoral neck fracture surgery.Methods A total of 150 elderly patients with femoral neck fracture treated in Pucheng County Hospital of Fujian Province from September 2019 to September 2022 were selected and randomly divided into two groups by simple random sampling method,namely,the experimental group(75 cases)and the control group(75 cases).The control group received routine nursing,and the experimental group received cluster nursing on this basis.The selfrating Depression scale(SDS),self-rating anxiety scale(SAS),pain assessment scale(VAS),nursing satisfaction and complications were observed and compared between the two groups before and after nursing.Results After intervention,the scores of SDS(29.20±1.40)and SAS(28.90±4.70)in the experimental group were significantly lower than those in the control group(40.40±4.30)and(40.30±4.50),and the differences between the two groups were statistically significant(t=30.333,21.457,P<0.05).At 24 h and 72 h after operation,the VAS scores of the experimental group(3.20±0.70)and(2.80±0.60)were significantly lower than those of the control group(6.60±0.80)and(4.60±0.50),and the differences were statistically significant(t=39.173,28.226,P<0.05).The satisfaction of the experimental group was 96%,and that of the control group was 84%,and the difference between the two groups was statistically significant(χ^(2)=6.000,P<0.05).The complication rate of experimental group was 4.00%,which was significantly lower than that of control group 25.33%(χ^(2)=13.636,P<0.05).Conclusions Cluster nursing can effectively improve the poor psychological state of elderly patients with femoral neck fracture surgery,relieve pain symptoms,reduce the risk of complications,and improve nursing satisfaction,which is worthy of clinical application.
