摘要
目的 :分析上海市嘉定区江桥镇初产妇产后抑郁现状及相关影响因素,为一线城市初产妇产后抑郁防治提供依据。方法 :选取2020年11月2日—2021年11月21日期间居住在上海市嘉定区江桥镇的所有初产妇作为调查对象进行问卷调查,问卷内容包括一般资料、爱丁堡产后抑郁量表和社会支持评定量表。结果 :324名初产妇的抑郁发生率为12.3%,抑郁组初产妇的主观支持、客观支持、对支持的利用度和社会支持现状得分均低于非抑郁组,差异均有统计学意义(P <0.05)。多因素logistic回归分析结果显示,产前焦虑状况、照顾状况、不良孕产史、产后睡眠状况、宫缩到分娩时间及社会支持现状均为对初产妇产后抑郁具有统计学意义的影响因素(P <0.05)。结论 :社区医务人员在开展产后家庭访视时,应重点关注初产妇的心理状况,发现异常应及时予以干预,避免和降低产后抑郁的发生。
Objective: To analyze the prevalence of postpartum depression and the influencing factors among primiparas in Jiangqiao Community of Jiading District, Shanghai and to provide theoretical basis for the prevention of postpartum depression in primiparas in first-tier cities. Methods: All the primiparas, who lived in Jiangqiao Community of Jiading District, Shanghai from November 2 2020 to November 2 2021 were selected as the subjects of the questionnaire survey, and the contents of the questionnaire included general information, Edinburgh postpartum depression scale and social support rating scale. Results:Among 324 primiparas, the prevalence of postpartum depression was 12.3%, and the scores of subjective support, objective support, utilization of support and social support status in the depression group were lower than those in the non-depression group, and the difference was statistically significant(P<0.05). Multivariate logistic regression analysis showed that prenatal anxiety, nursing status, previous poor pregnancy history, postpartum sleep status, time from contractions to delivery, and social support status were all statistically significant influencing factors for postpartum depression in primiparas. Conclusion: The community maternal and child health workers should pay attention to the psychological status of primiparas when carrying out postpartum family visits, and the timely intervention should be given to avoid and reduce the occurrence of postpartum depression when abnormal psychological state is found.
