超声Graf法在婴儿发育性髋关节发育不良筛查中的应用效果研究 被引量:4

摘要 目的:分析在婴儿发育性髋关节发育不良(DDH)筛查中应用超声Graf法的临床效果。方法:选取2017年1月-2021年1月天水市第一人民医院收治的300例疑似DDH患儿为研究对象,均接受超声Graf法检查、临床查体及X线检查,以查体和X线检查诊断结果作为“金标准”,分析超声Graf法的诊断效果。结果:经超声Graf法检查,发现髋关节异常患儿17例,共28个髋关节。女性DDH患儿占比高于男性,臀位分娩患儿占比高于非臀位分娩,差异有统计学意义(P<0.05)。发育不良髋关节ɑ角比正常髋关节小,β角比正常髋关节大,差异有统计学意义(P<0.05)。以临床查体及X线诊断结果作为“金标准”,超声Graf法检查的敏感性为93.33%,特异性为100%,准确率为99.87%。患儿接受对症治疗后,超声Graf法复查可见,患儿经治疗后异常髋关节均转为正常。结论:超声Graf法诊断婴儿DDH的敏感性高,特异性强,准确率高,具有较高的应用价值。 Objective:To analyze the application effect of ultrasonic Graf method in the screening of developmental dysplasia of the hip(DDH)in infants.Methods:From January 2017 to January 2021,300 children with suspected DDH in Tianshui First People's Hospital were selected as the study subjects.All the children received Graf ultrasound,clinical physical examination and X-ray examination.The physical examination and X-ray examination diagnosis results were taken as the"gold standard"to analyze the diagnostic effect of ultrasonic Graf method.Results:17 children with abnormal hip joints(28 hip joints)were diagnosed by ultrasonic Graf method.The proportion of female children was higher than that of male children,and the proportion of breech delivery children was higher than that of non-breech delivery children,with statistically significant difference(P<0.05).Theɑangle of hip joint of developmental dysplasia was smaller than that of normal hip joint,and theβangle was larger than that of normal hip joint,with statistically significant difference(P<0.05).Taking clinical physical examination and X-ray diagnosis results as the"gold standard",the sensitivity,specificity and accuracy of ultrasonic Graf method were 93.33%,100%and 99.87%respectively.All children received symptomatic treatment,and the re-examination by ultrasonic Graf method showed that all children's abnormal hip joints turned to normal after treatment.Conclusion:For infants,ultrasonic Graf method has a high diagnostic sensitivity,strong specificity,high accuracy and high application value.
