摘要
目的观察千金苇茎汤中药离子导入辅助治疗慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)痰热郁肺证患者的临床疗效。方法将62例AECOPD痰热郁肺证患者,采用随机分组法分为试验组(脱落1例,完成治疗30例)和对照组(31例)。对照组患者采用西医常规治疗,试验组在对照组治疗方案基础上辅助给予千金苇茎汤离子导入疗法,每日1次,每次15~20 min,共治疗10 d。观察治疗前后两组患者中医证候总积分及各项积分、中医证候疗效、血白细胞(white blood cell,WBC)计数、血沉(erythrocyte sedimentation rate,ESR)、血清降钙素原(procalcitonin,PCT)及C-反应蛋白(C-reactive protein,CRP)、慢性阻塞性肺疾病评估测评问卷(COPD assessment test,CAT)评分的变化情况。结果两组患者治疗后中医证候总积分、各项积分,CAT评分均较治疗前显著减少(P<0.05),炎症指标水平均较治疗前显著降低(P<0.05);试验组患者中医证候总积分、各项积分,CAT评分减少程度以及炎症指标水平降低程度均较对照组更为显著(P<0.05)。试验组疗效优于对照组(P<0.05)。结论千金苇茎汤离子导入辅助治疗AECOPD痰热郁肺证,能够显著减轻临床症状、控制患者肺部炎症水平、改善患者生活质量。
Objective To investigate clinical effect of iontophoresis of Qianjin Weijing Decoction as an adjuvant therapy for acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with the syndrome of phlegm-heat obstructing the lungs.Methods A total of 62 AECOPD patients with the syndrome of phlegm-heat obstructing the lungs were randomly divided into experimental group with 31 patients(among whom 1 was lost to follow-up and 30 completed the treatment)and control group with 31 patients.The patients in the control group were given conventional Western medicine treatment,and those in the experimental group were given the adjuvant therapy with iontophoresis of Qianjin Weijing Decoction in addition to the treatment in the control group,15-20 minutes each time,once a day for 10 days.The two groups were observed in terms of total TCM syndrome score and scores of each item,treatment outcome of TCM syndrome,white blood cell count(WBC),erythrocyte sedimentation rate(ESR),procalcitonin(PCT),C-reactive protein(CRP),and COPD assessment test(CAT)score.Results After treatment,both groups had significant reductions in total TCM syndrome score,scores of each item,CAT score,and l evels of inflammatory indicators(P<0.05),and the experimental group had significantly greater reductions in the above indices than the control group(P<0.05).The experimental group had a significantly better treatment outcome than the control group(P<0.05).Conclusion As an adjuvant therapy for AECOPD with the syndrome of phlegm-heat obstructing the lungs,iontophoresis of Qianjin Weijing Decoction can significantly alleviate clinical symptoms,control the level of pulmonary infection,and improve patients'quality of life.
