摘要
目的:探究及观察J型针刀推切法在指屈肌腱狭窄性腱鞘炎中的应用效果及对甲襞微循环的影响。方法:将2019年1月-2022年1月南昌市洪都中医院的120例指屈肌腱狭窄性腱鞘炎患者根据随机数字表法分为A组、B组和C组,每组40例。A组进行封闭治疗,B组进行小针刀治疗,C组则进行J型针刀推切法治疗。比较三组的临床疗效、治疗前后的疼痛情况[视觉模拟评分法(VAS)]、手指关节活动度及甲襞微循环指标(甲襞微循环加权积分、白微栓率、管襻畸形率)。结果:C组总有效率显著高于A组及B组,差异均有统计学意义(P<0.05);治疗前三组的VAS评分、手指关节活动度及甲襞微循环各指标比较,差异均无统计学意义(P>0.05),治疗后8周,C组的VAS评分、手指关节活动度及甲襞微循环各指标均优于A组及B组,B组则均优于A组,差异均有统计学意义(P<0.05)。结论:J型针刀推切法在指屈肌腱狭窄性腱鞘炎中的应用效果较好,可缓解患者局部疼痛及关节活动受限等不适,改善甲襞微循环状态。
Objective:To investigate and observe application effect of J-type needle knife push cutting method in stenotic tenosynovitis of finger flexor tendon and influence on nailfold microcirculation.Method:A total of 120 patients with stenotic tenosynovitis of finger flexor tendon in Nanchang Hongdu Hospital of Traditional Chinese Medicine from January 2019 to January 2022 were divided into group A,group B and group C according to the random number table,with 40 cases in each group.The group A was treated with block therapy,the group B was treated with needle knife,the group C was treated with J-type needle knife push cutting method.The total effective rates,pain situation[visual analogue scale(VAS)],range of finger motion and nailfold microcirculation indexes(weighted integral of nailfold microcirculation,white microthrombus rate and tube loop deformity rate)of the three groups before and after the treatment were compared.Result:The total effective rate of group C was significantly higher than that of group A and group B,the differences were statistically significant(P<0.05),the VAS scores,range of finger motion and nailfold microcirculation indexes of the three groups before the treatment were compared,the differences were not statistically significant(P>0.05),the VAS score,range of finger motion and nailfold microcirculation indexes of group C at eighth week after the treatment were all significantly better than those of group A and group B,those of group B were significantly better than those of group A,the differences were statistically significant(P<0.05).Conclusion:J-type needle knife push cutting method has good application effect in the treatment of stenotic tenosynovitis of finger flexor tendon,in that it can relieve the discomfort such as local pain and limited joint motion of patients,and improve the nailfold microcirculation state.
