摘要
目的 探讨电针联合补阳还五汤对颈椎间盘退变大鼠颈椎软骨终板血管芽的影响并分析其作用机制。方法 选取60只8~12周龄SD雄性大鼠随机分为假手术组、模型组、电针组、补阳还五汤组(中药组)、电针联合补阳还五汤组(联合组),每组12只,建立颈椎间盘退变大鼠模型,24 h后假手术组及模型组不采取任何治疗措施,电针组采用电针治疗,中药组给与补阳还五汤治疗,联合组采取电针联合补阳还五汤治疗。21 d后,比较各组大鼠X线评分、血管芽数目、软骨终板钙化层厚度及血管内皮生长因子(VEGF)、白细胞介素-1β(IL-1β)表达水平。结果 各组大鼠X线评分差异具有统计学意义(P<0.05),两两比较发现,其余4组均高于假手术组,电针组、中药组、联合组低于模型组,联合组低于电针组、中药组(P<0.05)。各组大鼠间软骨终板交界面血管芽数目、软骨终板钙化层及非钙化层厚度差异均具有统计学意义(P<0.05),其余4组血管芽数目、非钙化层厚度均低于假手术组,电针组、中药组、联合组高于模型组,联合组高于电针组、中药组(P<0.05);其余4组钙化层厚度均高于假手术组,电针组、中药组、联合组低于模型组,联合组低于电针组、中药组(P<0.05)。各组大鼠间软骨终板中VEGF、IL-1β表达水平差异均具有统计学意义(P<0.05),其余4组VEGF、IL-1β表达水平均高于假手术组,电针组、中药组、联合组低于模型组,联合组低于电针组、中药组(P<0.05)。结论 电针联合补阳还五汤可能通过下调软骨终板中VEGF、IL-1β表达水平,增加软骨终板内血管芽数目、降低钙化层厚度,发挥延缓颈椎间盘退变的作用。
Objective To investigate the effect of electroacupuncture combined with Buyang Huanwu Decoction(补阳还五汤)on vascular bud of cervical cartilage endplate in rats with cervical disc degeneration and analyze its mechanism.Methods Sixty male SD rats aged 8~12 weeks were randomly divided into sham operation group,model group,electroacupuncture group,Buyang Huanwu Decoction group(traditional Chinese medicine group),electroacupuncture combined with Buyang Huanwu Decoction group(combined group),with 12 rats in each group.The traditional Chinese medicine group was treated with Buyang Huanwu Decoction,and the combined group was treated with electroacupuncture combined with Buyang Huanwu Decoction.After 21 days,the X-ray score,the number of vascular buds,the thickness of calcified layer of cartilage endplate,and the expressions of vascular endothelial growth factor(VEGF)and interleukin-1β(IL-1β)were compared.Results The difference of X-ray score in each group was statistically significant(P<0.05).The X-ray scores of the other four groups were higher than that of the sham operation group and the scores of the electroacupuncture group,traditional Chinese medicine group and combined group were lower than those of the model group,and the scores of the combined group were lower than those of the electroacupuncture group and traditional Chinese medicine group(P<0.05).There were significant differences in the number of vascular buds,thickness of calcified layer and non-calcified layer in each group(P<0.05).The number of vascular buds and thickness of non-calcified layer in the other four groups were lower than those in the sham operation group,while those in the electroacupuncture group,traditional Chinese medicine group and combined group were higher than those in the model group,and those in the combined group were higher than those in the electroacupuncture group and traditional Chinese medicine group(P<0.05).The thickness of calcification layer in the other four groups was higher than that in the sham operation group,and the thickness of the electroacupuncture group,traditional Chinese medicine group and combined group was lower than that in the electroacupuncture group and traditional Chinese medicine group(P<0.05).The levels of vascular endothelial growth factor(VEGF)and interleukin-1β(IL-1β)in cartilage endplate of each group were higher after treatment(P<0.05).The levels of the four groups were higher than those of the sham operation group.The levels of the electroacupuncture group,traditional Chinese medicine group and combine group were lower than those of the model group(P<0.05).Conclusion Electroacupuncture combined with Buyang Huanwu Decoction may down-regulate the levels of VEGF and IL-1βin cartilage endplate.It can increase the number of vascular buds in cartilage endplate,reduce the thickness of calcification layer and delay the degeneration of cervical intervertebral disc.
